目的：本研究之目的在於比較髖關節置換手術後的病患，術後是否接受物理治療，其住院日數、出院時功能狀況之差異，及對出院後後續治療計畫之認知程度的不同。方法：以某醫學中心骨科病房接受髖關節置換手術之病患為對象，隨機分配至接受治療組與對照組；前者有21名病患，後者則有28名病患。主要之新發現：治療組病患平均住院日為10.9日，並平均接受了3.05次之物理治療，對照組之平均住院日則為11.6日，無顯著差異。在轉位技巧方面，治療組可獨立完成的人數百分比多於對照組，但無顯著差異。在行走能力的表現上，助行器的種類與高度是否合適的人數比例(p=0.000)、使用合於標準之步態行走者的比例(p=0.004)、行走耐力(p=0.014)等三項治療組明顯優於對照組。有關於各禁忌動作、日常生活中該注意的事項及出院後該做之運動項目，治療組病患了解的人數也都明顯多於對照組(p<0.05)。結論：住院期間物理治療對病患之功能恢復有正面之效果，今後應對兩組病患作長期追蹤，以更了解髖關節置換手術的病人在住院期間是否接受物理治療，對其長期預後之影響。

Purposes: The purposes of this study are to determine the effect of physical therapy on the length of hospital stay (LOS), functional status and the knowledge in contraindications and follow-up exercise program in patients with total hip arthroplasty. Methods: Forty-nine patients received total hip arthroplasty in a university hospital were　recruited in this study and were randomized into treatment (21 patients) and control group (28 patients). Results: The LOS were 10.9 and 11.6 days in treatment and control group respectively, and the patients in treatment group received post-op-erative physical therapy for 3.05 times on average. Greater percentage of patients can perform transfer　activities independently in treatment group than in control group, but thedifference was not significant. More patients in the treatment group walked with correct gait pattern (p =0.004), using appropriate walking aids (p=0.000) and better endurance (p=0.014). Besides, the patients in treatment group had better knowledge about the contraindications for total hipreplacement patients, post-surgery course and the follow-up exercise program.
Conclusion: Physical therapy during hospitalization really helps patients with total hip arthroplasty. Further study should emphasize on the long-term follow-up of patients' outcome to verify the significance of post-operative physical therapy for these patients.
