背景與目的:肌內效貼布(Kinesio® Tex tape)是一種宣稱可以促進淋巴循環,減少疼痛及改善微循環、有彈性的材質貼布,肌內效貼紮(kinesio taping)研究常應用在運動及骨科病患,但較少應用在偏癱的中風病人,因此本個案報告描述肌內效貼紮對於一名慢性中風病人的應用,並以連續測試方式比較個案在右腳赤腳、穿足踝副木(ankle-foot orthosis, AFO)、踝關節使用肌內效貼紮和同時穿AFO加上足踝肌內效貼紮這四種情況下,在行走速度之變化。方法:個案是一名16歲女孩,有慢性右半邊偏癱及感覺缺失的情形,可以獨立行走,在長距離行走時,右腳需穿戴前穿式AFO。讓個案在最舒服的速度下行走,比較她在赤腳、穿AFO、足踝肌內效貼紮以及同時穿AFO加上足踝肌內效貼紮這四種情況下,在行走速度變化。足踝肌內效貼紮的目的是減少步行時垂足及足內翻之現象。主要比較評量方式包括計時起走(timed up and go, TUG)及利用電子步道(GAITRite system, CIR Systems Inc., Havertown, PA),蒐集行走時的時間與空間步態參數。結果:個案在四種情形中,穿AFO加上足踝肌效貼紮的情形下行走速度最快,並增加了步長、步頻變快;在TUG為穿AFO情形下速度最快。結論:本個案呈現使用肌內效貼紮後的立即效果,可以提昇行走速度,但因長期效果未知,未來值得繼續探討。(物理治療 2009;34(5):304-309)

英文摘要

Background and Purpose: Kinesio® tape is an elastic material for taping which claimed to promote limbic circulation, relieve pain, and improve micro-circulation. The efficacy of kinesio taping for improving function in adults with orthopedic problems or in sport has received attention in recent years, but only limited report discussed the application of the kinesio taping in stroke patients. The aim of this case report was to describe the application of ankle kinesio taping on the affected side for a chronic stroke subject, as well as reported walking velocity among the four conditions (barefoot, ankle-foot-orthosis (AFO) only, kinesio taping only, and walking with AFO and kinesio taping). Methods: A 16-year-old girl with right chronic hemiparsis and sensory deficit. She was able to walk independently, and needed to wear right AFO for long distance walking. Her gait performances among four conditions, included barefoot, AFO only, kinesio taping only, and walking with AFO and kinesio taping of her right lower leg, were assessed. The kinesio taping was applied on the affected lower leg with intention to decrease occurrence of drop foot and foot inversion during walking. She was asked to walk at her comfortable speed on the GAITRite walkway for three trials for each condition. The measurements included the spatial and temporal parameters of the GAITRite walkway measure and the timed up and go (TUG) test. Results: The subject demonstrated fastest walk speed, along with improved stride length and cadence, while she had the right AFO and the kinesio taping. She completed
the TUG quickest in the AFO only condition. Conclusions: The immediate effect of kinesio taping, especially with AFO together, to improve walking speed was observed in this subject. However, the long-term effect was reminded unknown. Therefore, it will be worthwhile for investigating
the long term effectiveness. (FJPT 2009;34(5):304-309)
