本文描述照護一位護理之家住民害怕跌倒的護理經驗，個案因接連兩次跌倒造成龍骨骨折，產生了害怕跌倒的心理反應。只是護期間運用老人周全性評估技巧，評估個案生理、心理、社會及自我照顧能力等各個層面的狀況，並藉由傾聽及會談的方式進行主客觀資料的收集，發現個案健康問題為害怕跌倒及自我統合障礙。照護期間應用自我效能及行為改變理論引導措施執行，透過提供個別性的照護，在陪伴及鼓勵下，改善個案害怕跌倒問題，並進一步提升對自我的肯定。希望照顧經驗能提供專業照顧者照顧此類健康問題個案之參考。

This article describes the nursing experience of caring for a nursing-home resident with a fear of falling. The patient suffered hip fractures from two falls, and as a result developed an acute fear of falling.During the patient’s care period, comprehensive geriatric assessment was used to assess the patient’s physical functions, psychosocial functions, and self-care ability. Subjective and objective data were collected by listening and interviewing. The patient was found to have a fear of falling and selfintegration impairment. Self-efficacy theory and behavior-change theory were used to guide the implementation of the intervention.
Individualized care, companionship, and encouragement were provided. As a result of the care, the patient’s fear of falling decreased and self-affirmation increased.The study provides an example for professional caregivers taking care of patients with similar conditions.
