背景和目的:深層及淺層腰部多裂肌的收縮型態可用影像超音波評估,但尚未與實際肌肉的解剖位置比較。本研究的目的即在離體豬隻模型上比較深層及淺層腹部多裂肌之實際解剖位置及超音波影像。方法:以解剖腰部區域之豬肉確認深層及淺層腰部多裂肌肉走向,並且在解剖過程中放注入墨水在深層及淺層腹部多裂肌之間,按著以影像超音波追溯包覆深層及淺層多裂肌的筋膜,並以之前注入墨水處確認解剖位置。結果豬肉模型之深層多裂肌尾端連接關節囊外側,頭端連接兩節之上的脊突外側,淺層腹部多裂肌尾端連接關節囊外側,頭端連接三節之上之脊突外側。在超音波影像中,也已經注入墨水處重覆確認包覆深層及淺層腰部多裂肌的筋膜走向。結論:影像超音波可用來確認深層及淺層多裂肌的連接點及走向,並可呼應解剖情況。臨床意接:人類與豬隻的深層腹部多裂肌型態皆是連接兩節之間之關節翼及脊突,本研究方法的確立有助於未來以影像超音波尋找人類之深層腰部多裂肌。

英文摘要

Background and Purpose: The contraction of the deep (dMF) and the superficial layer (sMF) of lumbar multifidus could be evaluated by ultrasound image (USI). However, measurement of USI has not been confirmed by tracing real muscle attachment. This study is aimed to confirm the mensurement of USI of the dMF and the sMF by comparing with the result of dissection. Methods: One lumbar region of pork specimen was used to be dissected to confirm the architecture of sMF and dMF. During dissection, we injected some ink between the dMF and the sMF. We later identified the fascia boundary between dMF and sMF under USI by observing the position of the ink injection. Results: The dMF attaches to the lateral border of facet joint capsule caudally and to the lateral border of spinous process of two levels above rostrally. The sMF attaches to the lateral border of facet joint capsule caudally and the lateral border of spinous process of three levels above rostrally. Under USI, we traced the fascia which enwraps the dMF and the sMF and confirm the attachment by finding inkinjection madrance. Conclusions: The attachment of the dMF and the sMF could be identified using USI. The structure identified by USI is comparable with the real attachment. Clinical Relevance: The architecture of dMF in porcine demonstrated attachment of fascicles which run crosses two levels of facet joints. The result may be comparable in the finding of the dMF in human.
