目的：本文的目的為探討是否可以使用品管圈手法來提升物理治療紀錄完整率。方法：在台中市某區域醫院物理治療部門依據品管原則與概念，執行品管圈方法，發現物理治療評估單為該院目前物理治療紀錄不完整的主要問題，經由圈員腦力激盪找出要因並擬定改善對策，改善對策有：設計為簡單易寫的勾選式電子評估單、制定需完成物理治療病人的評估與治療計劃的時間及利用電腦保存物理治療紀錄。結果：物理治療紀錄的書寫完成率由實施前的2.0%，對策實施後大幅提升為40.4%。目標達成率為106.6%。結論與建議：運用品管圈活動可提升物理治療紀錄的完整率。為提供物理治療病患最佳的治療品質，完整的物理治療紀錄性的是需要物理治療師共同努力來推動。

Purpose: The purpose of this study was to apply the quality control circle (QCC) in a hospital's physical therapy department at Taichung. The goal of this QCC was to increase the completion rate of physical therapy record, which was initially found to be 2.0%. Method: We reviewed the problem and designed three methods to improve this situation. These methods are designing electrical evaluation form, stipulating the time to complete record and storing physical therapy record by computer. Result: The target achievement rate was 106.6%. Conclusion: The QCC method is efficient to increase the completion rate of physical therapy record.
