輪椅是肢體障礙兒童經常使用的移位輔具，除了可以增加兒童的移動功能、促進其他功能性技巧，並藉由擺位預防續發性的關節攣縮與骨骼變形。對於照顧者而言，輪椅可以方便日常生活照顧，並有效率且安全地移動兒童。本文報告一位十歲患有粒腺體病變的多重障礙兒童接受輪椅服務的流程，並探討影響身心障礙兒童輪椅服務流程的相關因素。這位十歲的個案因原有輪椅太小及功能不佳需重新量身訂作一台特製輪椅。服務流程中包含收集基本資料、了解使用者需要、建立可行目標、依據輔具需求進行身體功能與輔具試用等功能性評估、擬定工作流程及介入計畫、輔具製作與輔具適用性檢測、輔具使用訓練與成效評估以及後續追蹤輔具使用情況。專業團隊在提供輪椅服務時依標準服務流程可增進服務品質及杜絕不適用之輔具。此外在服務過程中家長的參與、充實產品資訊與輔具的檢核也都是影響目標達成的重要因素。

Wheelchairs are commonly provided for children with motor disabilities, they can be used to improve children’s mobility and acquisition of other functional skills. Secondary joint contractures and deformities can also be prevented through special designing and proper positioning in wheelchairs. A proper wheelchair promotes the child's care from the perspective of caregivers. Parents can thus move their children more efficiently and safely. The wheelchair service delivery process for a ten-year-old boy with multiple disabilities resulted from Leigh disease was reported. The boy's original wheelchair was out of growth. A custom-made wheelchair was needed to fit his and the family's needs. The wheelchair service delivery process included collecting initial data, identifying user's needs, having realistic goals, providing special consideration based on the characteristics and needs of the user, performing assessment related to the assistive device, generating a working hypothesis, planning and implementing the intervention strategy, and providing training, outcome measure and follow-ups. The service quality can be improved with standard delivery process and unfit assistive devices can be avoided. Special considerations should be made for children. Parent participation, product information and examination for the fitness of assistive device are crucial to the attainment of service goals.
