背景及目的:排水測量法、周長測量法與組織張力測量法(tissue tonometry)皆是淋巴水腫常用的臨床評估方式,具有臨床重要性,但探討其信度之文獻不多,且組織張力測量法之程序,缺少標準化。本研究旨在探討排水測量法、周長測量法、標準組織張力測量法及修正組織張力測量法之信度,並針對後兩者作比較。方法:兩位施測者以排水測量法、周長測量法測量32隻上肢;以標準組識張力測量法及修正組識張力測量法測量20隻正常上肢各三部位。結果:排水測量法與周長測量法之測試者間與測試者內再測信度係數皆高達0.99(P<0.05)。兩種組織張力測量法變異係數介於0.04~0.26;施測者內變異係數較施測者間小,而前臂測得之變異量小於手背與手指;修正組織張力測量法之變異小於標準組織張力測量法法。結論:排水測量法與周長測量法應用在正常受試者身上,具有很好的再測信度,二種組織張力測量法之施測者內變異量皆小,又以修正組織張力測量法較精確,但需耗費較多人力與時間。

Background and Purposes: Water displacement, circumference measurement and tissue tonometry are important clinical evaluation methods for the lymphedemous limbs. However, there are insufficient studies to investigate their reliability, and the measuring procedures are not standardized. The purposes of this study were to investigate the reliability of these measuring methods and to compare two kinds of tissue tonometry methods using standard tonometer and modified tonometer. Methods: This study recruited 32 upper extremities for water displacement and circumference measurement and 20 upper extremities for tonometry. Two investigators involved in each evaluation method. Results: The intra-rater reliability and inter-rater reliability of the water displacement and circumference measurement were high (ICC=0.99, p<0.05). The coefficient of variation of both tonometry methods were ranging from 0.04 to 0.65. Intra-rater coefficients of variation were lower than that of inter-raters'. The coefficients of variation were lowest measured at forearm in comparison of at dorsum of hand and finger. Coefficients of variation of modified tonometry were lower than standard tonometry. Conclusion: Water displacement and circumference measurement have good reliability in measuring healthy subjects. The modified tonometry was more accurate, but took much time and effort to use it.
