目的:本研究旨在長期照護個案有關輪椅、柺杖、助行器、下肢裝具等類行動輔具之實際使用狀況。方法:透過焦點團體、分析、德菲法專家效度等方式,設計行動輔具使用者問卷。並於某醫學中心方便取樣12名(9男、3婦;平均48.2歲)初測受測者以兩名施測者訪以分析者間信度。再由台閩地區長期照護機,共111名訪員參與本調查,成功完成468位長期照護個案之訪問。結果:除了關於輪椅尺寸目測韙信度較差外,問卷之施測者間信度良好(平均weighted Kappa=0.88)。受訪者平均65歲,目前使用輪椅者67%、柺杖及助行器類47%、下肢裝具9%。經醫療人員建議選購或指導使用者約三至五成。曾有受傷或跌倒經驗者約15%(輪椅)、46.5%(柺杖及助行器類)及55%(下肢裝具)。能自行出門使用效能工具者約有7%(輪椅)、37%(柺杖及助行器類)及41%(下肢裝具)。結論:本問卷包括輔具使用時活動情形、照顧者使用方法等韙,研究結果可供輔具設計、輔具訓練等之參攷。

英文摘要

Purpose: To survey the current usage conditions of locomotive assistive devices (wheelchair, cane, lower extremity orthoses) for long term care clients. Method: A usage survey questionnaire was designed based on focus group discussion and video analysis. The Delphi method was used to verify the face and content validity. Inter-rater reliability was tested with two testers interviewing 12 subjects (9 males, 3 females, average 48.2 years). Then 29 long-term care facilities with 111 interviewers at counties composed of more than 10000 older residents assisted in the survey. A total of 468 long term care clients who were using locomotive assistive devices completed the interview. Results: The inter-rater reliability for the questionnaire was satisfactory (average weighted Kappa=0.88) except a few questions regarding visual assessment of the appropriateness of the size of the wheelchair. The participating long term care clients averaged 65 years old and 67% were using wheelchairs, 47% were using canes or walkers, and 9% were using lower limb orthoses. Less than 50% of subjects have received medical opinions on selecting assistive devices or usage of the devices. Use-related injuries or accidents were not uncommon (15% in wheel chair users, 55% in orthoses users). Seven percent of wheelchair users and 41% of lower limb orthoses users were able to take public transportation or taxi independently. Conclusion: The clinicians should be aware of the problems revealed by this study for better care of users of assistive devices.
