目的:早期療育的服務流程包括:宣導、篩檢、發現、通報、鑑定、評估、轉介、療育安置等部分,在各個層面都需要醫療、教育與社政方面的合作與連結,才能夠提供個案整合性的早期療育服務。跨專業團隊合作模式被認為是最能夠提供早期療育整合性服務方案的模式之一,本計劃希望藉由跨機構間早期療育服務協調機制的建立、跨專業團隊合作專業知能的研習,跨專業團隊合作評估模式的示範,提供南投縣醫療、社政與教育機構一個運作跨專業團隊合作模式的運作經驗,期望未來能夠建立南投縣早期療育專業團隊,提供發展遲緩兒童連續性、整合性的早期療育服務方案。方法:本計劃總共舉辦跨機構早期療育服務協調會議一場、早期療育宣導活動在職訓練一場、早期療育專業人員培訓課程兩場、跨專業團隊評估示範會議八場。結果:跨機構早期療育服務協調會議,使得大部分的醫療院所能夠了解此計畫對於發展遲緩兒童個案的重要性,並且建立後續合作的共識。三場教育訓練的内容與主題,主要是針對社區化早期療育服務及跨專業團隊合作模式所需要的專業知能進行研習活動。八場跨專業團隊評估示範會議分別於南投縣的四個醫療分區輪流舉辦,結果顯示大部分參與的專業人員,都認為這樣的服務模式能夠促進自己跨專業領域的認知及團隊合作技巧,擔任評估示範的專業人員,都能掌握跨專業團隊評估的內涵,完整的執行評估。受訪者大部分也認為此一模式所完成的療育建議,對於小朋友來說非常適切可行。在整體收穫上,大部分受訪者認為非常滿意,並且願意在未來的工作環境中嘗試運用此一模式,提供發展遲緩兒童整合性的療育服務。結論:研究計劃結果建議未來政府應積極建構偏遠地區整合性的早期療育服務體系、提供專業人員完整的教育訓練以及鼓勵區域性跨專業早期療育團隊的成立與運作。

英文摘要

Purpose: The provision of early intervention for children with developmental disability requires the integration of medical, social and educational resources. Trans-disciplinary team model has been recognized as the best model for the provision of integrated early intervention service. The purpose of the project was to establish the early intervention team model in Nantou county that provides continuous, integrated, community-based early intervention services. Methods: In this project, one coordinating meeting was held to reach a common consensus for procedures between agencies from medical, social welfare and educational sectors. One workshop was held to propagate the early intervention model. Another two workshops for early intervention professionals were held to provide the knowledge base and training for the operation of trans-disciplinary team approach. They agreed that the content of the workshops is appropriate for the implementation of trans-disciplinary team model in early intervention. The practical evaluation for children with disability under trans-disciplinary team model was demonstrated through eight meetings in four medical districts in this County. Results: Most participants concurred that the demonstration would increase the skills for team approach. They also agreed the early intervention program recommended by the team was appropriate for child. Most participants were satisfied with the outcome and would try to set up this model in their facility in the future. Conclusion: This project suggests that the local government should coordinate the early intervention system, provide training workshops and encourage the initiation and operation of local early intervention team.
