背景與目的:Hislop等人以能連續墊腳尖20次為蹠屈肌之正常級的標準。作者已於2001年用相同的實驗方法,證實台灣相似年齡層(<60歲)的人不易達到此標準次數。由於年齡與能完成動作次數的能力有極密切的相關性,因此本實驗繼續探究大於60歲以上的國人所能完成的單腳站立墊腳尖次數,再結合先前之實驗,即可了解各年齡層的國人蹠屈肌能完成墊腳尖之次數。方法:以40位60~80歲,下肢沒有任何傷害且不需任何輔助器即可獨立行走的健康者為研究對象,男女各20名共40名。以慣用腳為測試腳,在單腳站立姿勢下連續做墊腳尖的動作,做到電子量角器記錄墊腳尖的蹠屈角度,一小於第一次墊腳尖的蹠屈角度的二分之一時即停止測試。又以身體平衡控制力及受試者本人主觀的腿部無力感等因素協助判斷測試動作是否標準。結果:大於六十歲以上的國人,單腳站立墊腳尖動作最多只能完成7下。百分之八十的人能完成兩下。年齡和能完成的次數在統計上有顯著相關性。結論:以20下的單腳站立墊腳尖次數當作蹠屈肌的標準值,對大於六十歲以上的年長者來說是不適當。建議60歲以上的人,以能連續做2次即可評定為正常級。(物理治療2003;28(6):293-299)

英文摘要

Background and Purposes: The normal grade of Calf muscle strength testing is the capability to perform plantar flexion for 20 repetitions in single leg standing position (Hislop, 1995). The results of previous serial article indicated that there is discrepancy between such criteria and the capability of age matched population (i.e., <60 years old) in Taiwan. Because of the highly correlation between the age and the number of movement repetitions, population old than 60 years old might also have difficulty to perform 20 repeated plantar flexions. The purpose of current study is to apply the same testing procedure and examine if such criteria is appropriate to the people even older than 60 years old. Methods: 40healthy participants were recruited (20 men, 20 women; averaged age: 69.1 years).The repetitions of dominant legs plantar flexions were recorded and the successful trial should meet three criteria: 1) the range of motion of the trial is greater than half of the first trial;2) the movement was performed with balance; 3) there is no subjective weakness complaint. Results: Only one participant could perform 7 repetitions and 80% of participants had 2 repetitions. In addition, there are high correlations between age and number of repetitions (r=0.46). Conclusion: Setting 20 repeated plantar flexions as normal grade is not appropriate for population in Taiwan. Based on the results, 2 repetitions are suggested to be normal grade calf muscle strength in people older than 60 year old. (FJPT 2003; 28(6):293-299)
