背景與目的:許多研究指出物理治療對尿失禁有明確的療效,但國內鮮有類似的物理治療經驗。林口長庚醫學中心婦女泌尿科自民國89年起對尿失禁者進行積極性的物理治療。本研究旨在探討接受物理治療之尿失禁者的尿失禁型態、嚴重度、骨盆底肌的肌力分佈情形,並瞭解不同年齡層間的差異。方法:本研究收集民國89年1月到民國90年年底間至林口長庚醫學中心婦女泌尿科門診的尿失禁個案共2,223人,以卡方檢定、t-檢定和無母數分析之(Wilconxon rank sums test),分析其中372 (16.7%)名接受積極的物理治療者之狀況,及老年與年輕兩組間之差異。結果,患者過半數年齡在35-54歲間,而10.9%年齡大於65歲。57.4%的患者懷孕次數超過3次,約1件患者生產過4次以上。年輕患者以應力性尿失禁居多,而老年患者以急迫性尿失禁為多數。陰道壓力測試則顯示老年患者的陰道壓力較年輕患者低。以PERFECT方法評估肌力顯示,老年患者在肌力、耐力、和肌肉快速收縮都明顯地較年輕患者差。結論:本研究資料顯示不同年齡層的尿失禁者之情形迥異,老年患者較年輕患者情況更為嚴重。

Background and Purpose: Previous research has shown that physical therapy is effective for treatment of urinary incontinence Similar approach is rarely used in Tai an Chang Gung Memorial Hospital in Linkou started an intensive physical therapy program for women with urinary incontinence since 1995. The purposes of this study were: (1) to investigate the type and severity of urinary incontinence, and the strength of the pelvic floor muscles in patients with urinary incontinence and received intensive physical therapy, and (2) to compare the differences between the young and elderly groups. Methods: Three hundred and seventy-two urinary incontinent patients ho received intensive physical therapy were selected as subjects in this study. The subjects were recrected from a 2223-case pool which consisted of patients ho came to OPD of urogynecology between Jan 2000 and Dec. 2001. The inclusion criterion as that the patients had entered the intensive physical therapy program for 6 to 12 weeks. The patients were categorized into young (age <65 years old) and older (age <65years old) group. Results: Only 35 of 322 patients (10.9%) ho received intensive physical therapy in our clinic were over 65 years old More than 50% of the subjects were between 35 and 54 years of age. Approximately half of the patients (57.4%) had pregnancy more than 3 times, and 27.7% patients had birth deliveries more than 3 times. Most patients (72.0%) in the young group had genuine stress incontinence; however, the majoriety of (61.0%) of the elderly group had urge incontinence. The vaginal pressure in the elderly patients as lower than the young ones. According to the PERFECT scheme, muscle power, endurance, and the fast contractions of the elderly patients were poorer than the young ones. Conclusions: There were significant differences in the types and severity of urinary incontinence between young and elderly groups. The elderly patients show ed more severe urinary incontinence & poorer strength of pelvic floor muscles than the young ones.
