目的:1.針對持續在接受肢體復健的發展遲緩兒童,評估其運動功能改善的情形;2.探討與其運動功能改善相關之影響因素。方法:共有10名物理治療師及3名職能治療師參與本研究;各治療師從其服務院所現有的治療個案中選取研究對象,使用粗動作功能評量(Gross Motor Function Measure, GMFM)量表進行2次運動功能評估,期間間隔3個月。治療師並取得三類的相關資料,包括家庭人口學資料、研究對象的個人資料及其接受治療的情況。此外,記錄研究對象的治療出席日數以計算兩次GMFM評估期間之平均每週治療頻率。各治療師須參與使用GMFM量表的重測信度及施測者間信度分析。結果:共收錄44名腦性麻痺兒童,平均年齡為46個月。成對t檢定結果顯示兩次GMFM總分在統計學上有顯著的差異。以區辨分析法對多項變數進行分析,結果為母親的學歷、家中手足數,及出生體重此3項因素具有重要的區辨貢獻量。結論:44名持續接受肢體復健的發展遲緩兒童,在相隔3個月的期間其運動功能在統計學上有顯著的進步。研究對象是否呈現出具有臨床意義的運動進步量,顯示取決於母親的學歷、家中手足數及出生體重此三項變數。

英文摘要

Purposes: The first purpose was to evaluate the motor function improvement in children with motor developmental delay who have continuously received the intervention of rehabilitation. The second purpose was to investigate significant variables correlated to the Improvement of motor function. Methods: Ten clinical physical therapists and 3 occupational therapists participated in this study. They recruited the study subjects from their Current caseload and evaluated the subjects' motor function by using the gross motor function measure (GMFM) twice at a three-month interval. The therapists also took three categories of the subjects' information, which were the family demographic data, the subjects' personal data, and the data regarding their rehabilitation intervention. The test-retest and inter-rater reliabilities of administrating GMFM were examined. Results: Forth-four children with cerebral palsy were recruited (mean age: 46 months). Paired t-test revealed the difference between the two GMFM total scores was significant. The discriminant analysis identified three variables: the educational level of the mothers, the number of sibling and the subjects' birth body weight, that were important for distinguishing the subjects whether they had achieved the clinically important motor function Improvement. Conclusions: The motor function of the 44 subjects had improved statistically significantly over the three-month treatment period. Clinically motor improvement appeared to be determined by the variables of mothers' educational level, number of sibling and birth body weight.
