中文摘要

肌電圖及咬力分析常作為探討咬合板治療的工具。但咬合板對下顎運動速度的影響情況則不知。本研究的目的即在於探討顯額障礙的人口中置入咬合板對其下顎運動速度的影響。二十名顛額障礙患者(十六名男性,四名女性,年齡分佈在二十一歲至二十六歲)參與此實驗。利用MyotronicK-6系統觀察咬合板放入口內前後下顎運動速度的變化。每位受測者在熟悉整個測試情況下經指導後儘可能作出最大且最快的開閉口動作。其十次連續的開閉口運動速度在咬合板放置前及放置後均記錄儲存並以下列變項作為分析比較標準:(1)最大開口速度,(2)平均開口速度,(3)最大閉口速度,(4)平均閉口速度,(5)牙齒接觸前的最終閉口速度。其連續十次開閉口速度的平均值以變異數分析(ANOVA)來作比較咬合板對運動速度的影響。資料經分析後顯示出在咬合板放入口內後其最大閉口及平均閉口速度均減少(P<0.05),而在開口速度及牙齒接觸前最終速度則不受咬合板置入的影響(P>0.05)。在此實驗情況下,結論為咬合板能減緩下額運動的閉口速度。本研究須要進一步作長時間的觀察才能真正了解咬合板產生治療效果時的生理機轉。

英文摘要

Background: The effect of the occlusal splint has been studied extensively by using electromyography (EMG) and bite force analysis. Previous report had elucidated the effects of occlusal splint on the velocities of the mandibular movement in non-temporomandibular disorders (TMD) subjects. Aim: To determine the influence of occlusal splint on the velocity of mandibular movement in TMD subjects. Methods: Twenty TMD patients (16 males and 4 females, ranging from 21 to 26 years old) were observed by using Myotronics Kinesiograph K-6 Model for the measurement of jaw motion velocity before and immediately after occlusal splint insertion. Each subject was instructed to open and close the mouth as wide and fast as possible. Ten continuous open-close strokes were recorded and processed to evaluate the following parameters: (1) the maximal opening and closing velocity, (2) the average opening and closing velocity and (3) the maximal terminal velocity 1 mm before tooth contact. Mean velocity of ten consecutive movement strokes was compared with the result by ANOVA. Results: Analysis of data indicated that the maximal and average closing velocities were reduced after occlusal splint insertion (P<0.01). Neither the opening velocity nor the maximal terminal closing velocity was affected by splint insertion (P>0.05). Conclusion: Under the condition of this experiment, the velocity of mandibular closing movement phase decreased immediately after occlusal splint wearing in TMD patients.
