此研究調查之目的,藉由分析98年度潛水人員因罹患減壓病至本院接受高壓氧治療之病人資料,以了解目前基隆地區潛水人員之潛水方式與減壓病罹病率之相關性,與高壓氧治療之探討。98年度本院治療潛水人員減壓病共計218人次,依下列因素做分析:(1)性別:男生居多,佔92.2%。(2)年齡:以30-50歲居多,佔73.4%。(3)職業:病人職業為漁民、水下工程工作者、休閒潛水及地形觀察,其中以漁民居多,佔61.5%。(4)潛水年資:平均10.0±8.6年,最少1年以下,最多40多年。(5)重複潛水次數:平均2.61±1.4次,最多達8次。(6)潛水深度:平均29±9.3公尺,最淺5公尺,最深50公尺。(7)常用供氧方式:潛水人員常用供氧方式以水肺居多,佔55.5%。(8)執行減壓程序:100 %未執行減壓程序。(9)減壓病病症:以第ㄧ型減壓病為主,症狀以關節痛最多。由分析結果顯示,目前基隆地區減壓病的病人以漁民為主,其特點為年紀較大、潛水
年資長,雖對當地潛水環境熟悉,但為捕撈漁獲,其潛水深度較深、水下停留時間較久、且未遵照減壓程序上升,故罹患減壓病的機會較高。在減壓病治療方面,治療表表5所需耗費的人力較少及時間較短,亦能有效改善症狀,可作為臨床上治療第一型輕症減壓病優先之考量。

英文摘要

The purpose of this study is to investigate the association between the morbidity of decompression sickness and the diving behavior of divers, and the hyperbaric oxygen treatment of decompression in Keelung, by analyzing the data of decompression sickness patients in our hospital.
The total number of enrolled patients was 218 in 2009. The study results showed as following: (1). Sex: male patient (92.2% of total patients); (2). Age: most in range from 30 to 50 years old (73.4% of total patients); (3). Occupation: most was fisherman (61.5%); (4). Average diving seniority: 10.0±8.6 years; (5). Average repeated diving number: 2.61±1.4 times one day; (6). Average diving depth: 29±9.3 meters; (7). Air supply: most was scuba (55.5%); (8) Decompression procedure: 100% patients didn’t obey the correct decompression procedure; (9). Type of decompression sickness: most was type I and joint pain was the frequent symptom. The results revealed that fishermen were the major population of decompression sickness patients in Keelung. They dove longer and deeper than other groups of divers. The fisherman got higher incidence of decompression sickness due to ignoring a correct decompression procedure.
Regarding the treatment of decompression sickness, the US Navy table 5 is the treatment of choice for type I decompression sickness.
