多數根管治療完的牙齒,都需要根柱及〈或〉冠心來補強牙齒的結構,以獲得較佳的狀態來回復其功能與健康。如何選擇一個理想的根柱及冠心是一項技術與藝術,本篇文章將近來幾年的文獻探討不同的因子,歸納整理成易於遵循及參考的條列,其中考量的四大方向因子包括:1.牙齒的本身解剖構造(anatomy)、殘餘牙冠的結構(remaining coronal structure)、牙根長度、根管型態;2.壓力(應力stress)、扭力(torsional force)、靜水壓(hydro static pressure)、牙齒裂開的抵抗力;3.根柱本身的因子-寬度、長度、設計、材質,以及材料的相容性;4.根柱冠心之囝持力(retention)、再治療或修正的考量、美觀(esthetics)的要求。彙整如上述,以利醫師在臨床上能簡單有效率地思考及應用,並能輕鬆地向病人解釋,以使得良好及成功的治療。
Most of the endodontically treated teeth requires post and core for the reinforcement of the tooth structure to restore their function and health. It is an art and science to select the ideal post and core. Accroding to the recently reviewed articles, the authors conclude the four elements which includes 1. the anatomy of the tooh 2. the 3-dimensional force applied on the tooth 3. the biochemical and the hiomechanical factors of the post and core 4. the other factos such as the retention and the retreatahility and the esthetic considerations. So we present the above for the doctors to think and apply easily and efficiently to achieve successful clinical outcomes.
