目的:本研究目的在檢驗緊縮型頭痛(Tension-type headache,TTH)患者在非頭痛發作時期,休息與壓力兩種狀況下之顱周肌及自主神經活動,以驗證TTH之心理病理假設。方法:採二因子混合設計,組間因子包括慢性緊縮型頭痛(chronic tension-type headache,CTTH)、週期性緊縮型頭痛(episodictension-type headache,ETTH)、及非頭痛控制 (control,CON) 組;實驗操弄為組內因子,分別是基準期、心算壓力期、以及回復期。採個案控制研究(case control study)法,共28名女性受試者資料:CTTH組4人;ETTH組13人;CON組11人,平均年齡20.46±1.35歲。心理生理測量指標包括前額肌活動電位與自主神經反應。結果:總結本研究結果,在休息與心理壓力狀態下,CTTH患者相對於ETTH組與健康控制組,有較高的前額肌活動電位;此外在心理壓力下,CTTH患者也有較高的副交感神經反應。結論: 本研究推論CTTH患者比ETTH患者或控制組,易將環境視為威脅、不安全,持續出現前顱周肌緊繃現象,因而容易引發頭痛症狀。再者,CTTH患者,面對壓力事件,更易表現出無助或無望的情緒與生理狀態。

英文摘要

Objective: The purpose of this study was to examine the reactions of the frontalis muscle and autonomic nervous system under psychological stress in tension-type headache patients.Method: The study consisted of four participants with chronic tension-type headaches(CTTH), 13 participants
with frequent episodic tension-type headaches(ETTH), and 11 participants headache-free(control). All participants were female college students. The participants’ activities of frontalis muscle and autonomic
nervous system were measured during the baseline and psychological stress(arithmetic test)stages continuously. It was found that, during both the baseline and mental stress stages, the CTTH group expressed higher frontalis muscle activity than both the ETTH and control groups.Result: Contradicting our hypothesis, during the mental stress stage, the CTTH group expressed higher parasympathetic nerve reactivity than the other two groups.Conclusion: Conclusion: The results suggest that individuals with CTTH may feel more threatening and unsafe in their daily life than those with ETTH and normal controls. Furthermore, they must be more likely to experience feelings of helpless and hopeless while they confront life stresses.
