背景 糖尿病照護除了治療外,最重要的是糖尿病病人要能自己做好自我管理;因為,超過90%的糖尿病照護,需要他們在居家環境中自行完成。對專業人員來說,最具挑戰性的工作,是如何讓病人做好自我管理,因為涉及的因素多且複雜。因此,如何能在有限的門診時間內,讓專業人員很有效能的評估病人的糖尿病自我管理能力,並依此規畫出屬於糖尿病病人的個人照護計畫,是糖尿病照護重要的一環。目的 透過雙向翻譯,發展自覺糖尿病自我管理量表(Perceived Diabetes Self-Management Scale, PDSMS)中文版並測試其信、效度。方法 本研究採橫斷式研究設計,分為兩個階段,第一階段為雙向翻譯建立中文版自覺糖尿病自我管理量表。第二階段為專家內容效度的檢測。於量表確認後,採立意取樣168名糖尿病個案進行量表信度測量。兩個星期後再從原本168名的個案中隨機挑出37名個案進行再測信度。結果 中文版的自覺糖尿病自我管理量表共八題項,本文以因素分析法求得良好之建構效度,將量表分為「正向語詞」和「負向語詞」。量表之內容效度指標值為.75;Cronbach's α為.926,其內在一致性佳;兩週後使用組內相關係數檢測再測信度為.966(p值<.001)。結果顯示本量表具良好之信、效度,可做為自覺糖尿病自我管理的評估工具。結論/實務應用 台灣的糖尿病衛教人員,向來缺少評估個案的適當工具,而此自覺糖尿病自我管理量表,可以對糖尿病患者的自我管理能力,作一簡單而快速的評估,以提供符合個別化的糖尿病照護計畫。除此外,此量表也被建議可用於其它的慢性病病人,作為評估其自覺自我管理能力的工具,以便提供合宜的照護計畫。

英文摘要

Background: Aside from receiving proper medical treatment, patient self-management is the most important part of diabetes care. Over 90% of diabetes care is conducted in the patient's living environment. It has been a consistent challenge for healthcare professionals to make patients follow self-management instructions as well as to deal with the many complicated factors involved. Within the limited time permitted during clinic visits, health professionals must effectively assess patient self-management abilities and design a proper, patient-specific self-care program.Purpose: This study developed a Chinese version of the Psychometric Properties section of the Perceived Diabetes Self-Management Scale (PDSMS) using two-way translation and then examined reliability and validity.Methods: This cross-sectional study was executed in two phases. In the first, the researchers translated the English version PDSMS into Chinese using forward and backward translation. In the second, three healthcare professionals expert in diabetes care verified content validity. The study recruited 168 participants with diabetes, and two weeks later 37 were re-tested to check instrument reliability.Results: The Chinese version PDSMS consisted of 8 items. Factor analysis obtained good construct validity, yielding four "positive words" and four "negative words." The content validity index was .75, Cronbach's α was .926 (good internal consistency), and the intraclass correlation coefficient of test-retest reliability was .966 (p-value <.001) two weeks after the first test. Results suggest that the scale can be used as a valid tool to assess diabetes self-management due to its high reliability and validity.Conclusions/Implications for Practice: Diabetes educators suffer from a lack of appropriate assessment tools to measure diabetes self-management effectively. Devising a personal care plan is thus critical to proper self-care. The PDSMS developed in this study can help achieve this purpose. This tool may also be applied to chronically ill patients in order to evaluate self-management capabilities prior to designing individualized care plans.
