背景 醫護人員因受通報態度的影響,對醫療異常事件經常有少報的情形發生,病人安全通報與醫院中所有員工都有關,但過去鮮少人進行醫院員工對病人安全通報態度的研究。目的 探討醫院員工基本特性、通報經驗及通報認知與病人安全通報態度的關係。方法 本研究採橫斷性研究設計,問卷調查期間為2007年9月3日至17日,研究對象為南部某區域醫院醫療人員,總共發出857份自填式問卷,共回收405份有效問卷。問卷內容包括基本資料、病人安全通報經驗、通報認知、通報態度,以t檢定、ANOVA檢定和皮爾森相關等統計方法進行資料分析。結果 服務單位在通報態度上有顯著差異,護理單位通報態度分數最低,病人安全的教育訓練應採行以單位為基礎的案例分析,以利改善整體的工作單位對於病人安全的通報態度。主管階級在通報態度上則有顯著性差異,通報認知與通報態度有統計上顯著的正相關,讓員工瞭解醫院對於病人安全事件管理和改善的過程,應可以促進員工正向的通報態度。結論 研究結果乃做為管理者針對病人安全通報系統建置的參考,以增加醫院員工的通報行為,達到從經驗中共同學習的目標。

英文摘要

Background: Physician and nurse underreporting of incident events is attributed to improper attitudes toward reporting responsibilities. Patient safety is a core responsibility of all hospital professionals. Little research to date has explored the attitudes of healthcare professionals to patient safety.Purpose: This study examined the relationship amongst personal characteristics, reporting experiences and knowledge, and reporting attitudes in hospital professionals.Methods: Researchers conducted a cross-sectional hospital-based survey at a region hospital in southern Taiwan between September 3rd and 17th, 2007. A total of 405 of the hospital's 857 employees agreed to participate. Two sample t tests, ANOVA, and Pearson's correlation were used to analyze the data.Results: Healthcare professional reporting attitudes differed significantly amongst different working departments. The nursing department had the lowest reporting attitude scores. In response to findings in the initial phase, researchers designed and helped implement an incident clarification and feedback mechanism in each department to improve incident reporting. Significantly different attitudes were identified amongst positions, but no differences were found in terms of education level. Knowledge of patient safety reporting related positively to attitude. Patient safety management should be made the responsibility of employees in order to promote positive reporting attitudes.Conclusion/Implication for practice: This result provides information about establishing patient safety reporting to hospital managers in order to enhance reporting rates and achieve the overall goal of learning from experience.
