本病例報告提出一燒傷患者，因為上下顎局部缺牙，影響咀嚼功能而計畫在上顎施以可撒式局部義齒方式進行贋復，但是因為臉部及頸部燒傷疤痕組織收縮，造成下唇及嘴角部分下拉且無彈性，而口輪匝肌也呈現環周收縮，使得病患開口困難且狹小，一般模托無法置入口內，一次取得完整模型以製作義齒，患者配戴的過程也會受到侷限，考慮印模準確性及患者配戴假牙的方便性，本例採以修正模型印模技術以及改變活動假牙的設計來重建。印摸過程首先需利用可拋式儲備模托，修整成可置入口內大小，涵蓋牙齒部分，再以藻膠印模；游離端缺牙區利用印模油灰初步印樣，製作石膏模型後再組合製作全顎研究用模型，並藉此設計製作個別模托進行修正模型印模，以記錄出缺牙區及軟硬將之假牙覆蓋區域的精準模型。

A case of 57 y/o male patient with burn scar who had partial edentulous over both maxilla and mandible which impairs chewing function. The treatment plan is maxillary removable partial denture as prosthetic rehabilitation. But the burn had resulted in lower lip depression and lack of flexibility. The patient also had mouth opening limitation because the orbicularis oris muscle is circumferentially contracted. It is impossible to get impression with traditional stock tray for fabricating pros thesis and it will then be difficult for the patient to wear the denture. Considering impression accuracy and convenience of wearing prosthesis, we utilize altered cast impression technique and modify the design of removable partial denture to rehabilitate. First, trimming disposable tray flange to appropriate size of the mouth, which involves only the teeth part, and take impression with alginate. Then preliminary impression of free end edentulous area with silicon putty was made. Finally, we assembled the full arch study cast from the two separate parts In order to get an accurate cast of edentulous area and soft/hard palatal tissue where removable partial denture involved, we designed individual tray which was then made from the study cast to take altered cast impression. Notice that when making individual tray, it has to carve groove on palatal side of teeth area to prevent impression material overflow into teeth area which will affect the accuracy of the impression.
