中文摘要

目的:使用高解析平行孔洞式準直儀、應用田口方法評量核醫腦部灌注斷層掃描檢查之最佳調整參數。方法:本實驗在評估核醫腦部灌注斷層掃描檢查過程中,歸納出放射性製劑之活性強度、病患每一個角度平面影像之攝取時間與攝影掃描轉動角度等三種影響最大之控制因子;每一個控制因子皆使用三個變動水準,故採用田口方法中之L9(34)直交表並使用Hoffman腦部假體掃描來評估每組不同調整參數下的影像晶質。核醫腦部灌注斷 層掃描共9組27張影像優劣之評估,由二位核醫科專科醫師審視其影像對稱性及均句度給予分數,最後再經由信號/雜訊比與變異數分析統計分析結果。結果:依據田口方法望大特性S/Nratio '得到核醫腦部灌注斷層掃描之最佳化造影條件為:放射性製劑之活性強度2.5mCi、病患、每一個角度平面影像之攝取時間的秒與攝影掃描轉動角度3度。結論:應用田口方法評量核醫腦部灌注斷層掃描之最佳調整參數研究中,主要取決於造影時間及角度之選擇;由控制因子分類觀念得知,放射性製劑之活性強度對影像品質有一定影響力,但由於本實驗驗證中發現如使用1.8mCi仍可獲得相當品質之影像,故可經由調整此類控制因子來使放射性製劑成本降至最低。
