為初步瞭解接受居家護理服務個案的用藥情形，採描述性研究設計，對象來自屏東縣某一獨立型態之居家護理機構，針對在家接受居家護理服務的個案共30位，於2008年1月至11月，利用結構式問卷，收集個案與主要照顧家屬之基本資料，目前用藥處方之顆數、種類、每日服藥次數，經藥師及居家護理師評估造成用藥安全問題之項目。結果發現30位個案，主要診斷以腦血管疾病與糖尿病居多，日常生活活動功能以巴氏量表評估平均為10.2分，完全依賴者達八成，每一位個案均需家人協助用藥，平均用藥顆數為14.1±6.5顆，每日服藥次數平均為四次，平均用藥種類為6.3±2.6種；依用藥種類來看，以使用消化性用藥最多，次為心血管與中樞神經系統用藥。藥師與居家護理師評估發現有13位個案（43.3%）有用藥安全上的問題共20項，以副作用居多，如腹瀉、便秘、腸胃道出血、血壓與血糖過高或過低等。依研究結果擬定居家護理實務應用策略，並建議藥師應加入居家護理團隊，共同來促進用藥安全，讓每位居家個案能安心用藥。

OBJECTIVE : The aim of this study was to assess medication used by home healthcare nursing residents. METHODS : Descriptive study design was used to collect data about medication prescription which were extracted from medical records using standardized extraction procedures. SETTING : One of the urban home healthcare organizations in Pingtung City, Taiwan. PARTICIPANTS : Home healthcare patients admitted to participating organization during Jan 2008 and Nov 2008. RESULTS : The average numbers of medications used by home healthcare patients among 30 respondents were 14.1±6.5 drugs. 6.3±2.6 type of medications by category were taking by respondents ; up to a maximum of 9 types of drugs per person were used and also accompanied most side effects, such as diarrhea, constipation, gastric and intestinal ulcer, unstable blood pressure and blood sugar level. The frequency of medications admitted for most subjects was four times per day. The medication for G-I system were most commonly prescribed follow by cardiovascular medication. CONCLUSION : Medications are critical to the health of older people who used the home healthcare services. For those patients with chronic illnesses, medications were used to control symptoms and cure disease. This study suggests that home healthcare organization should include pharmacist to improve pharmaceutical care and medication safety use. Improving the safety of using medications, the home nursing practice application strategy should be embraced.
