本專案主要目的是在提昇單位病患於出院後返回長照機構時交班內容之完整率，期望藉由加強單位與長照機構間交班之完整性，以使病患得到持續性、一貫性的照護品質。經查檢表監測及結構性問卷分析結果，發現影響出院後交班過程完整性之原因有:1.交班單內容不完整2.未明訂標準流程3.未於前一天派車導致病人延誤出院時間4.新進人員對出院流程不熟悉。本專案小組透過文獻蒐集及品管圈運用，經由下列四個方案:1.製定內容完整之交班單2.明定制式出院交班標準流程3.接道預約出院單即完成派車手續4.舉辦在職教育，並利用晨會加強對交班流程之認知等介入後，單位出院病患交班過程之完整率，由改善前之32.4%提昇至改善之95.2%，顯示專案改善成效顯著。

The purpose of this project was to promote completeness of nursing shift change during patient discharge to a long-term care center. Through enhancing completeness of nursing shift change between family medical ward and long-term care center, the researchers wished to offer excellent nursing quality of sustainability and consistency for patients. After analyzing the results of check lists and structured questionnaire, the factors of completeness of nursing shift change included:1.content of nursing change-of-shift report was incomplete;2.no standard flow chart;3.lack of arrangement for carring patients before their discharage;4.novices felt nufamiliar with the process of discharge. The researchers applied the method of quality control circle and found that there were four appropriate interventions:1.to draw up a complete nursing change-of-shift report;2.to set up a standard flow chart of discharge;3.to arrange for carrying patient after their planning to discharge;4.to hold related in-service education and with the help of morning to enhance the cognition of nursing instruction.Owing to the improvement of this project, the completeness of nursing shift change increased from 32.4% to 95.2%.
