一位八十歲慢性阻塞性肺部疾病的男性患者，因肺炎住院後不久，不幸因咳嗽，痰多黏稠，不易咳出，引起呼吸道阻塞，呼吸喘促，爭診再度住進內科病房，胸部x-光片檢查發現罹患肺炎，肺水腫，肋膜積水，隨因并發急性呼吸衰竭轉入加護病房，經積極積搶救治療，給予氧氣補給，抽痰及抗生素控制感染，但因呼吸衰竭，患者一直靠人工呼吸哭維持呼吸，于是實家屬要求會診中醫。中醫會診時，患者神識不清，眼閉不開，需靠鼻胃管餵食及呼吸器、抽痰器輔助，患者呼吸費力，肌虜甲錯，大骨枯槁，大肉陷下；四肢不時抽動，痰多黏稠，不易抽出，脈寸口左右弦急無根，合谷、太難、太衝脈浮現，證屬腎氣大虛，危急之候，故以景岳大補元煎力挽腎之元氣。患者終于在中西結合治療下，恢復了呼吸的生理功能。

A case of eighty years old male with chronic obstructive pulmonary disease was admitted due to suffer from pneumonia in rescent. He was just discharged a short time, unfortunately he was readmitted via emergency into Internal Medicine because of cough, sputum viscosity and difficult cough caused respiratory air way obstruction and breathing of shortness. Chest X-Ray examination demonstrated that he suffered from pneumonia, lung edema, and pleural effusion. As fast as, he was complicated with acute respiratory failure and was transfered into intense Nursing Care. He was resuscitated with 02 inhalation, extracted sputum and antibiotic controlled infection, but his respiratory failure could not be spontaneously recovered. So his family asked consult the diagnosis and treatment of Chinese Medicine. He was loss of consciousness, both eyes closed, naso-gastric tube fedding, respiration with artificial machine and assist of extracted vacuum when the patient was consulted the-Chinese physician. His breath was shortness and difficult, skin color incrusted, big bone desiccated and large muscle collapsed, and four extremities often tremor. His sputum was viscosity and difficult to extract. His inch pulse was showed wiry tight and empty. The pulse of Ho gu (LI-4), Tai xi (K1-3), and Tai chong (LV-3) were floating according to the pulse method of Three Position and Nine Indicators. The identified diagnosis of Chinese Medicine was Kidney qi vacuity severity pattern. The identification pattern was demonstrated that the patient was on critical. The treatment of Chinese Medicine was used Da Bu Yuan Jiari (Major Origin-Supplementing Brew) by Jing Yue Quan Shu to reserve origin-qi of the Kidney. At last, the patients physiologically respiratory function was recovered through Chinese Medicine and Western Medicine combined treatment.
