A 68-year-old male without muscular symptoms or signs underwent technetium-99m methylene diphosphonate bone scintigraphy as a part of malignant melanoma work-up. The scan results showed extensive whole-body skeletal muscle uptake and bright visualization of bilateral kidneys without standard definition of the skeletal system. These unexpected scintigraphic characteristics led to a working diagnosis of rhabdomyolysis with acute renal failure. Upon reviewing his food and medication history, he admitted ingesting one regimen of phytotherapy to remedy his melanoma. Although bone scintigraphy is used for evaluating the skeletal system, explicit interpretation of extraosseus tracer uptake may be of diagnostic value in some cases.
