本研究目的在對住院中輕度與中度失智老人提供懷舊團體治療的活動,經由介入性活動設計,期能提升長者言語表達能力,改善憂鬱情緒,增加社交互動,減少認知功能的退化。採單組前、後測設計的類實驗研究法,以立意取樣的方式,於北部某家精神科專科醫院的老人精神科病房進行研究,測量工具為簡短智能測驗和老年憂鬱量表。本研究分別進行二梯團體,每梯團體為每週一次連續六週的活動介入,全程參與者共有17位。結果顯示研究對象平均年齡74.06±5.84歲,女性居多,教育程度多以國小以下居多數,已婚者居多,但其中鰥寡者佔多數,且和與子女住在一起者有10位。以魏氏符號等級排序檢定比較懷舊團體介入效果,研究對象的認知功能的改變無顯著的效果,但憂鬱症狀經過介入後有顯著改善的效果,根據研究者的觀察,個案參與團體後減少憂鬱情緒、維持認知功能和增加言語表達、社交活動的能力。本研究結果可提供老年精神衛生護理在設計團體與安排照護活動時重要的參考依據。

英文摘要

The purpose of this study was to investigate how reminiscence therapy can be applied to elderly patients with mild and moderate levels dementia. Intervention were designed to increase verbal and emotion expression, improve depression and mood, increase social interaction, and delayed of degeneration of cognition function. A single-group pretest-posttest design in a quasi-experimental study was employed. The sampling was drawn from the psychiatric elderly ward of one northern Taiwan psychiatric hospital. The instruments used were the Mini Mental Status Examination (MMSE) and the Geriatric Depression Scale (GDS). Each group participated in six activities over the six-week period; a total of seventeen elderly patients participated fully in two groups. According to results of this study, the average age of subjects was 74.06±5.84 years old. Most of subjects were females who had received below an elementary school level of education. Many of them were married, a great number of them were widows, and ten subjects lived with their children. Based on a Wilcoxon signed-ranked test comparison of the effects of reminiscence therapy, cognitive function was not significantly improved, but the symptoms of depression symptoms in our subjects showed statistically significant improvement. Moreover, the subjects showed maintainance of cognitive function and increased verbal communication and social interaction ability in the group process. The study will be provided as an important reference to aid in designing group therapy and arranging nursing activities in elderly psychiatry nursing.
