本文描述協助一位乳房惡性腫瘤病患行左側乳房全切除術後處理生理及心理問題之護理過程。護理期間自98 年7 月12 日至7 月25 日，照護過程中藉由觀察、會談技巧、Gordon 十一項健康功能評估模式收集資料，確立個案護理問題為：疼痛、身體心像紊亂、焦慮及主要照顧者抉擇衝突。筆者運用持續性、完整性之護理過程，協助個案渡過腫瘤術後生理危機，並為個案安排復健活動、心理建設及提供出院準備服務措施，使個案的護理問題於出院時獲得改善，恢復最佳的身體狀況，並以正向表達持續追蹤治療的意願。期許此篇個案報告能提供護理人員未來在照護此類患者之參考。

This paper describes the nursing process of assisting a breast cancer patient who had undergone left-side mastectomy to handling physiological and psychological health problems. The nursing care period started from July 12th to July 25th 2009. During the process of care, through observation, interview techniques, Gordon 11 functional health assessment data collection, this study had confirmed the nursing problems of this case were: pain, body image disturbance, anxiety and major caregiver choice conflict. With using of continuing and integrating nursing process,
the author assists the patient to successfully go through physiological crisis caused by cancer surgery, arranging rehabilitation activities, and psychological building measures for the case so that the patient’s nursing problems can be improved while being discharging from the hospital, then recovering and back to the best physical condition, and positively expressing willingness of keeping tracking sequent treatment.
It is expected that this case report may provide nurse personnel reference for caring similar patients in the future.
