這位62歲女性病患，於94年時因爲疲勞倦怠、體重明顯下降10公斤及咳嗽有血而到西醫求診，雖初步診斷爲支氣管擴張症，但其血象異常卻未被進一步探查其病因及診察，而錯失早期診斷早期治療的先機。患者因HCV在其他醫院腸胃科說診及治療，但其肝功能未見改善，而改求診於嘉基中醫部。中醫初診時醫者診察其肝功能明顯異常且AFP高達197，遂立即轉診到腸胃科做進一步CT檢查。CT檢查結果顯示肝硬化，脾腫大，並且在肝門脈，腹主動脈及鼠蹊部有許多淋巴結出現。血腫科醫師在做完腫瘤指標及頸部CT後，排除Lymphoma，而高度懷疑是慢性林巴性白血病，其後在骨髓檢查後獲得確診是慢性淋巴性白血病。患者於97/9就診時主訴疲勞倦怠、頭暈、多次抱怨小腿骨頭痠痛，身熱多汗、怕熱、手心熱、顴紅赤，偶爾皮下瘀血，身疲體重明顯下降，診其舌紅絳質乾少津，脈弦細數，雅論是骨蒸潮熱，邪毒浸淫留於骨髓，進而耗血傷陰而致病，所以用秦艽鱉甲散來除骨蒸清虛熱，兼以填補精髓及涼血清熱。患者經過中西醫合併治療後，其白血球總數由14000下降到5000左右，而Segment亦由17%→29%→39%，整體而言其血象獲得改善。由本病例可以看出：慢性林巴性白血病在西醫治療時，以中醫藥來協同治療能夠控制及治療慢性林巴性白血病，明顯使其血象獲得改善，因此大大的提高慢性淋巴性白血病忽者的生存率及生活品質。

This 62 year-old female patient suffered from fatigue, body weight loss 10kg and hemoptysis since 2005. Then she visited the doctor of pulmonary department, and the initial diagnosis was bronchiectasis. Unfortunately, her abnormal laboratory data of blood counting was missed, so the further evaluation and early diagnosis were also lost, flue to the abnormal liver function test and general malaise, she visited our Chinese medicine department and asked for our combined treatment on 97/6. The Chinese medicine doctor checked the liver function test and significantly high level of alpha-fetal protein, and then he decided to refer this patient to the gastrointestinal department for CT scan. The CT scan found that there were multiple enlarged lymph nodes in hepatic hilum, pararotic region, splenic hilum and mesentery region. Otherwise, the oncologist had checked tumor marker and neck CT scan, and then the doctor excluded the lymphoma and highly suspected of the chronic lymphocytic leukemia. Finally, the diagnosis of chronic lymphocytic leukemia was confirmed by bone marrow biopsy. This patient complain of fatigue, general malaise, dizziness, leg soreness, hot sweating, hot palm, red cheek, skin ecchymosis and body weight loss. According to the Chinese medical theories, this chronic lymphocytic leukemia is belonging to the area of phlegm node, scrofula and malignant mass. The mechanism of this disease is due to the condensation of disturbed phlegm, the stasis of poisoned heat in the bone marrow, and it will exhaust the Yin finally. Chin-Chiu-Peih-Chia-San has the function of nourishing yin, clearing heat and hot blood, resolving phlegm and supple the bone marrow. After the integrated treatment of chemotherapy and Chinese medicine, the patient’ blood counting had improved. In this successful case, we found that the combination of Chinese medicine and chemotherapy was a good treatment for chronic lymphocytic leukemia. This combined treatment would successful control the chronic lymphocytic leukemia, improve the blood counting, and highly increase the survival rate and the quality of life for the chronic lymphocytic leukemia patient.
