Spontaneous subcapsular renal or perirenal hemorrhage is uncommon and is seldom suspected clinically. Bilateral spontaneous renal hemorrhage and cases in which a causal etiology fails to be identified are even rarer. A 32-year-old man without the history of trauma, a fall, or any invasive renal procedure experienced progressive bilateral flank pain and abdominal pain for 10 days. Detailed imaging modalities demonstrated the presence of bilateral subcapsular renal hematoma with no identifiable underlying pathological conditions. The patient was treated conservatively and followedup by ultrasonographic and computed tomographic scans until the hematoma had almost completely been reabsorbed. No surgical intervention was needed, and the patient recovered well.
