目的:發展及評估短期骨盆底肌肉訓練模式對婦女尿失禁之成效。材料與方法:採單組前後測之類實驗設計法,從民國92年2月到93年2月止,於某醫學中心診斷為應力性尿失禁、膀胱過動症及混合性尿失禁者為本研究之個案,同意接受8週骨盆底肌肉訓練模式,共有35位。用指診、生理回饋儀、簡版症狀困擾量表 (Short-form UDI)、簡版生活影響量表 (Short-form IIQ) 評值治療效果。結果:治療結果發現以指診及生理回饋儀測得個案骨盆底肌收縮力量都有進步,指診分數由訓練前7.1 ± l.7進步到8.5 ± 1.3,而生理回饋儀器所呈現的骨盆底肌肉收縮力量,亦從訓練前的27.3 ± 13.9進步到39.3 ± 16.3毫伏特,皆達統計顯著差異水準。症狀困擾量表的6個項目中,每項都獲得改善,其中4項達統計差異水準。生活影響量表的7個項目,每項前後測都達統計顯著差異水準,顯示尿失禁影響生活的程度獲得緩解。有34.1%個案對訓練效果感到非常滿意,45.7%感到滿意。年齡、產次、經期狀態及尿失禁種類,則與治療結果無顯著相關。結論:運用骨盆底肌肉訓練模式可以有效增強骨盆底肌的肌力,改善應力性尿失禁、膀胱過動症及混合性尿失禁婦女的困擾程度,明顯降低尿失禁對生活的影響以及身心的衝擊。

英文摘要

OBJECTIVES: The aims of this prospective study were to develop and evaluate a short-term training program to strengthen the pelvic floor muscle in women with urinary incontinence.MATERIALS AND METHODS: A 1-group, pretest-posttest, quasi-experimental design was utilized. The study sample of 35 female patients with urodynamic stress incontinence (n=10), overactive bladder (n=15), and mixed incontinence (n=10) was recruited from February 2003 to February 2004. All patients consented to and followed an 8-week training program. The efficacy of treatment was evaluated by vaginal palpation, a biofeedback instrument, the short-form Urogenital Distress Inventory (UDI-6), and the short-form Incontinence Impact Questionnaire (IIQ-7).RESULTS: After completing the 8-week training program, subjects' pelvic floor muscle strength had significantly increased from 7.1 ± 1.7 before training to 8.5 ± 1.3 after training. Similarly, pelvic floor muscle action potentials significantly increased after training, from 27.3 ± 13.9 to 39.3 ± 16.3 μV. Symptom distress had significantly improved for 4 of 6 items assessed by the UDI-6. Quality of life, assessed by the IIQ-7, also significantly improved, with 34.1% reporting being very satisfied and 45.7% satisfied with the training effect. No outcome measure was found to have been associated with subjects’ characteristics, including age, parity, menstrual status, or type of urinary incontinence.CONCLUSIONS: Pelvic floor muscle training can increase the pelvic floor muscle strength and improve the incontinence-specific quality of life for women with an overactive bladder or stress or mixed urinary incontinence.
