腎細胞癌的病患在診斷時大約有23％到30％已有遠處轉移，而腎上腺就是常見的轉移器官之一，回顧文獻記載有關於腎細胞癌合併對側腎上腺轉移者並不多見，其中又以同時性發生為多，而異時性單一對側腎上腺轉移之病例又更罕見。我們報告一例62歲男性在腎細胞癌術後將近6年發生單一對側之腎上腺轉移之罕見病例。術後輔助治療的角色也將進一步討論。

The adrenal gland is among the 5 organs most frequently involved with metastases of renal cell carcinoma (RCC). The majority of contralateral adrenal metastases from RCC involve synchronous metastasis. Fewer cases have involved metachronous solitary contralateral adrenal metastasis from RCC. We report on a 62-year-old male patient with solitary contralateral adrenal metastasis which occurred 6 years after a radical nephrectomy. He received a subtotal adrenalectomy and showed no evidence of disease after 3 years of follow-up. The role of adjuvant therapy is also discussed.
