An inguinal orchidectomy is the standard treatment for an intratesticular mass suspected of being malignant cancerous, however, a high proportion of patients with benign lesions also undergo this surgery because there is no certain preoperative diagnosis. Using ultrasonography and magnetic resonance imaging (MRI) of the testes may allow us make a correct diagnosis preoperatively, which led to organ-preserving surgery for this young male who had 2 epidermoid cysts in 1 testis.
