中文摘要

腦血管疾病是國人健康的大敵,除了是前三大死因外,更是青壯年人口失能與殘障的一大主因;而且其嚴重性隨著國內老年人口增加,有上升的趨勢。而最常見的腦血管疾病就是顱內動脈瘤破裂所導致的蜘蛛膜下腔出血(Subarachnoid Hemorrhage,SAH)。SAH死亡率極高,即便存活病患也往往留下嚴重後遺症。病患若同時有後天血管壁的硬化、老年退化性血管病變等,更增加了顱內動脈瘤的可能性。因此積極控制或治療前述的致病因素最直接的作法就是找出顱內動脈瘤所在位置,施以適當的處置,以防範未然。本研究目的是對於動脈瘤患者進行術前的多切面電腦斷層血管造影術(Multi-detector Computer Tomography Angiography,MDCTA)與三維旋轉血管造影術(Three Dimensional Rotation Angiography,3DRA)檢查,明確診斷出動脈瘤的位置、形狀、大小來做為術前的評估,並探討其應用之價值。

英文摘要

Cerebrovascular disease is a huge health impact for general population in Taiwan. It is one among the top three leading causes of death and contributes to a major cause of disabilities in young population. It has been observed that the severity of the condition increases as well as growing of the elderly population. One of the most common cerebrovascular diseases is subarachnoid hemorrhage (SAH) caused by ruptured intracranial aneurysm. Aneurysmal SAH is associated with high mortality rate and severe sequela. The incidence of intracranial aneurysm may increase in patients with acquired atherosclerosis and degenerative change of vessels. In order to prevent aneurysmal SAH, we have to identify the location of intracranial aneurysm and treat it appropriately. The aim of this study is to detect exact location, shape, and size of aneurysms by MDCTA (Multi–detector Computer Tomography Angiography) and 3DRA (Three Dimensional Rotation Angiography) before (surgical or radiological) intervention. We also discuss the clinical effects of these examinations.
