背景:脊髓損傷為人生遭遇之重大災難事件,損傷後個案面臨身體殘障、創傷帶來心理衝擊的問題。 目的:本研究旨瞭解脊髓損傷個案身心適應之生活經驗。 方法:採質性研究法,以立意取樣方式,從脊髓損傷協會徵求10位願意分享經驗之脊髓損傷個案,經其同意後,填妥知情同意書,以訓練訪員方式,至個案家中或與其約定訪談地點,利用開放式訪談指引,進行一對一的深度訪談。訪談內容徵求研究對象同意全程錄音,並將所得資料轉為逐字稿;再以內容分析法進行資料分析與歸納。 結果:研究結果可概括四項主題:(1)脊髓損傷的震驚與措手不及:災難瞬間降臨、身體功能喪失的認知不足;(2)未來的日子要如何過:否認脊髓損傷的事實、期待奇蹟出現;(3)陷入絕望的深淵:創傷深烙著生命;獨自隱忍痛苦的煎熬;生命秩序的混亂;(4)對生存意義之省思:哀慟過去,試著面對殘障的身體;自我接納及成長等四項經驗主題。 結論:本研究結果有助於護理人員進一步瞭解脊髓損傷個案身心適應之生活經驗,作為照護之參考。

英文摘要

Background: Spinal cord injury is a disaster that results in physical disability and has a major psychological impact on the victims. Objective: This study aims to explore the lived experiences in physical-psychological adaptation of cases with spinal cord injury. Methods: We conducted a qualitative study that employed purposive sampling to recruit 10 participants with spinal cord injury who gave consents to share their lived experiences. Using an open-ended interview guide, a single investigator conducted one-to-one interviews with the participants at their homes or other places they preferred. The interviews were tape-recorded for subsequent transcription. We used the content analysis method to analyze the interview contents and make induction. Results: We found four main themes of the lived experiences of spinal cord injury cases, including: (1) unpreparing for the spinal cord injury: striking of the disaster and under-recognition of the loss of physiological functions; (2) wondering how to spend the rest of one’s life: denial of the fact of spinal cord injury with the hope of a miracle; (3) falling into the abyss of despair: imprinting of life with the injury, self-bearing agonies, and chaos of life; and (4) reflecting on the meaning of life: adaptation to physical disabilities, self-acceptance, and growth. Conclusions: This study offers useful information to nurses responsible for handling the lived experiences with physical-psychological adaptation of cases with spinal cord injury.
