在有外傷的情況下,進行手部的常規斜位攝影時,會增加溝通的難度,每個病人配合的程度以及放射師擺放的角度不一致,以致於影像上常呈現旋轉過度或旋轉不足的問題。應用一些支撐的輔具來提升影像品質,增加病人配合度,減少溝通上因難,讓影像品質的一致性,斜位的正確性及關節腔的呈現皆獲得提升。本研究共收集200例使用支撐的輔具攝影方式,另外收集353 例為以往未使用輔具的舊病例,將所得之影像一一檢視確認,其掌骨重疊和不得重疊的部位,以及關節腔的呈現,予以分析統計。手部斜位攝影的正確性之比例由43.6%提升至86%,放射師在使用輔助器具時方便性的滿意度達90%。由兩組數據分析得知,使用支撐輔具的攝影方式,可清楚的顯現其解剖構造,提升影像品質的正確性及穩定性,且與病人的溝通變得簡單明暸,大大增加便利性。

英文摘要

It is difficult to communicate with traumatized patient while positioning the hand for oblique view. Depending on the experience of individual radiological technician and the degree of patient cooperation, positioning problem of under- or over-rotation is unavoidable. With the use of some supporting aids, we can probably improve the image quality, reduce the inter-technician variability, and reduce the need for patient cooperation. In this study, we analyze the image quality of 200 cases with supporting aids and 353 cases without the supporting aids, and revealed that the good position rate is 86% for group with supporting aids, which is much better than the rate of 43.6% for group without the supporting aids. In 90% of circumstances, the technicians are satisfactory with the convenience of the supporting aids.
According to these results, we claim that with the use of supporting aids, we can improve the image quality and consistency, reduce the demand for patient cooperation, and increase the convenience for radiological technician while positioning the oblique view of hand for traumatized patient.
