中文摘要

本篇是探討一位個案因膀胱癌接受膀胱全切除合併人造新膀胱重建手術之護理經驗。在2009年2月21日至3月14日住院照護期間,個案在接受人造新膀胱重建手術後,除了面臨身心衝擊與調適外,同時也因為術後排尿功能上改變,而出現尿失禁情形。手術全期照顧期間我們經由觀察、會談、傾聽方式,並運用Gordon十一項功能性健康型態評估工具,對病人手術前後狀況加以整理分析,並確立護理問題包括有手術前期:焦慮;手術中期:體溫過低、潛在性危險傷害;手術後期:急性疼痛、排尿型態改變、身體心像紊亂等。照護期間藉由護理專業角色的發揮,運用傾聽及同理心,進一步建立互信關係,此外我們也提供個別性措施,教導膀胱訓練之技巧,建立其信心,使個案走出漏尿陰霾,正向面對疾病的衝擊,以達手術全期安全之宗旨,於此,謹將此護理經驗提出分享。

英文摘要

We reported an experience for a with urinary bladder cancer receiving cystectomy and neobladder reconstruction. This case hospitalized from February 21st to March 14th in 2009. After the surgery, the patient experienced psychophysiological impact as well as adjustment. He also had urinary incontinence due to voiding dysfunction. During the hospitalization, we used observation, interviewing, listening and Gordons' 11 functional health assessment tools to evaluate the patient before and after the surgery. The problems encountered by nursing care could be divided into pre-, intra- and post-operative period. Problems occurred in pre-operative period included anxiety. In the process of interoperation, patient faced hypothermia, and potential risk of surgery. As for post-operative period, acute pain, change in voiding pattern, physical and psychological disorders were included. During hospitalization, nurses specially trained with skills provided the patient with listening, empathy and build-up of mutual trust. Besides, individualized care and bladder training had also been provided in order to help the patient to face the impact of the disease optimistically and enhance his self-teem. That is the prime goal for intraoperative safety practice. Herein, we shared our experience in this article.
