本文描述一位23歲男性因血尿、高血壓入院，最後診斷為原發性膀胱嗜鉻細胞癌並接受膀胱全切除及迴腸膀胱造口手術之護理經驗。筆者於2010／7／1至2010／7／30照護期間，運用Gordon十一項功能性健康型態評估工具，經由直接護理、觀察、會談、身體評估等方法收集資料，發現個案有身體心像改變、焦慮和知識缺失等護理問題，經由提供整體性的護理措施並給予護理指導，協助個案對疾病進展的認知與心理調適，並接受身體外觀的改變，以增進自我照顧的信心及能力，藉由鼓勵個案主動學習自我照顧的知識與技巧，重建正向身體心像，以提升生活品質。

This article describes the nursing care experience on a 23-year-old male who got hospitalization due to hematuria and hypertension. He was finally diagnosed to have primary bladder pheochromocytoma, and then received operation of total cystectomy and ileal conduit creation. The author applied Gordon's 11 Functional Health Patterns on the patient. Information was collcied by nursing care, observation, interview, and physical examination from July 1st to 30th in 2010. According to nursing diagnosis, we discovered some problems of this patient, such as pain, anxiety, lack of insight, and change in body image. We provided the patient holistic nursing assistance and guidance to help him to realize the progress of disease, to adjust psychological status, to accept his body image change, and to promote the confidence and ability of self-care. To improve the quality of life, we encourage the patient to learn how to do self-care well and to rebuild the positive body image.
