An 87-year-old female with left breast cancer underwent F-18 fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) for staging. A distended urinary bladder with a clear distinction between the urine without F-18 FDG radioactivity on the top and F-18 FDG-loaded urine on the bottom was anecdotally found. The appearance of these imaging characteristics led to a manifestation of posterior bladder layering sign.
