臨床上，安格氏第二類第一型咬合不正的病患，往往必須以拔牙的治療方式來獲得良好的顏面外觀改善。針對這樣的病患，是否有其他的治療考量？本報告藉由配戴頭套，以高拉及頸拉這二種方式，一方面可將第一大臼齒作遠心移動，另一方面也可以控制其垂直方向的位置，並且運用切角式自鎖式矯正器（Axis self-ligating bracket）和Tip-Edge Plus矯正器共同的特點，將溝槽的轉角做切角式的設計，有效降低弓線與溝槽轉角在滑行時的磨擦力（binding force）。藉由配戴輕度力量的第二類橡皮筋（約60克），以不拔牙的治療方式，改善嚴重的水平覆蓋及顏面外觀，達到不錯的治療結果。

In clinically, the improvement of facial profile can be obtained by tooth extraction methods in Angle's class II division I malocclusion patient. But, is it the only one method? The report of taking headgear combined with high and cervical pull direction appeared to make upper first molar distal movement by controlling vertical position. The same properties of Axis self-ligating and Tip-Edge Plus bracket can decrease binding force by the slot corner design when sliding. The improved result of the excessive overjet and facial profile by taking class II light force elastic (about 60g) and non-extraction treatment methods are acceptable.
