遠端肱骨骨折在臨床上不算常見，但是治療上卻有其困難度，它常發生在骨質疏鬆的患者，也常常發生粉碎性骨折，其解剖位置也因為比較複雜而導致不容易固定；即便是以手術固定治療，仍然有一定比例的併發症，如關節僵硬，無力，疼痛等。為了達到良好的穩定，可活動並且不痛的關節，在手術前必須做好詳細的評估，包括患者狀況適不適合手術固定，皮膚及軟組織的狀況，開刀入口的選擇以及內固定器的選擇等。此案例是一位遠端肱骨粉碎性骨折合併髓內釘斷裂的患者，我們選擇Y型鋼板來做內固定，術後追蹤患者的恢復狀況良好，骨折也癒合，結果顯示使用Y型鋼板能給予遠端肱骨骨折患者良好的預後。

Distal humerus fractures clinically are not common, but they remain some of the most challenging injuries to manage. They are commonly multifragmented, occur in osteopenic bone, and have complex anatomy with limited options for internal fixation. Treatment outcomes are often associated with elbow stiffness, weakness and pain. To achieve a painless, stable, and mobile elbow joint, pre-operative evaluation should be performed thoroughly including medical condition, the soft tissues’ condition, selecting a surgical approach, and selecting an implant for fracture fixation. Our case, a patient with a history of left humeral shaft fracture, suffered from comminted fracture of left distal humerus complicated with broken intramedullary nail. We performed internal fixation with Y plate for this patient and she recovered well with bony union. This result revealed that the Y plate can give a good outcome to a patient with distal humerus fracture.
