「國際健康功能與身心障礙分類系統（ICF )」及其兒童與青少年版（ICF-CY)影響台灣之醫療與社會 福利系統很大。2007年公布之「身心障礙權益者保障法」以ICF為導向。因此，2012年7月起身心 障礙鑑定新制中之醫療鑑定由專業團隊進行’內容包括ICF身體功能/構造與活動/參與等代碼。 身心障礙鑑定之流程分三步驟：即醫療檢查(b/s)、功能評估(d)、及需求評估。醫療人員負責醫療 檢查與功能評估，社工人員負責初步需求評估，並據以核發身心障礙證明，其上註記有身心障礙 類別、等級、以及交通相關福利。領有身心障礙證明者後續若有居家照顧、輔具與就業復健需求 時，社會福利單位會再組專業團隊，以進行詳細的需求評估，並提供需要的服務。在身心障礙權 益保障法通過之前’台灣物理治療師已經開始推廣ICF理論架構，且積極投入新制身心障礙鑑定 準備階段的相關活動。由2008年開始政府部門、非政府部門及學術單位都投入相關的準備工作， 包括研究計晝的補助、翻譯、評量工具之設計、評量流程之建立、相關人員的訓練以及研討會之 召開。本文以活動與參與的第四章「ICF動作量表」為例子，説明發展評量工具的過程，如何選取 相關的ICF代碼，定義限定值及人員訓練等等。有關於ICF-CY運用及在未來的身心障礙相關福利 的規劃’也加入討論。（物理治療2013;38(1):1-15)

The International Classification of Functioning, Disability and Health (ICF) and its children and youth version (ICF-CY) have changed the medical and social welfare service systems in Taiwan tremendously. According to the People with Disabilities Rights Protection Act promulgated in 2007, since July 2012，the disability evaluation has to be based on the ICF framework. In Taiwan, the disability eligibility system in Taiwan is a 3-step procedure (Medical Examination (ICF b/s), Functional Assessment (ICF d), and Needs Assessment (ICF e)). The health authority is responsible for conducting the process of disability certificate accreditation and the social welfare authority for providing the services needed to the persons with certificates. After assessment by health professionals, a medical evaluation report will be sent to the local health and social welfare departments. Then, the professionals in the social welfare system do brief Needs Assessments in order to issue disability identification with the information of disability category and severity level. Before the Act passed, the physical therapists in Taiwan started to embrace the ICF framework, and have participated in ICF/ICF-CY related activities for the preparation of the Act implementation. To implement that Act, a series of preparatory work inside the government system, non-government organizations, and academic societies began in 2008. The preparatory activities include: fighting for research funding, ICF documentation and measurement tool translation, development of measurement tools, evaluation establishment, staff training, and conferences of ICF. Another purpose of this article was to introduce assessments of Activities and Participation by using the ICF Mobility Scale as an example, to present the process of preparation (i.e. how to select codes and how to assign qualifiers) and its content. Issues regarding the implementation of ICF/ICF-CY and plans in the future are also discussed. (FJPT 2013;38(1):145)
