發展性協調不良(Developmental coordination disorder,簡稱DCD)兒童因動作困難常導致活動與參與的限制,甚至會影響社交的表現。本個案報吿目的乃利用「個案處理模式」(Patient/Client Management iModel)和「國際健康功能與身心障礙分類系統一兒童及青少年版」(ICF-CY )模式’應用 於一名DCD個案。其過程包括由物理治療評估、分析個案問題 '並報吿介入後的成果。本個案是 一位5歲9個月女生,其認知發展於魏氏幼兒智力量表的全量表智商為88,屬於正常範圍中下程 度,然於「兒童動作評量表」總分落在5百分等級,符合DCD的診斷標準。本文利用「個案處理模 式」之檢查 ' 評估 ' 診斷 ' 及預後等步驟並以ICF-CY架構分析其各面向之問題。依據分析’設定 減少跌倒次數、生活自理與增加親職能力為目標並擬定治療計畫。物理治療介入兩個月後’個案 與家長之目標大多達成。本文因此建議,物理治療師依據「個案處理模式」和ICF-CY模式為DCD 兒童設定功能取向目標及介入,可以達到預期的治療效杲。(物理治療2013:38(1 ):62-68 )
Children with developmental coordination disorder (DCD) are often limited in activity participation and social interaction due to movement difficulties. The purpose of this case report was to apply the Patient/Client Management Model (CMM) and International Classification of Functioning, Disability and Health-Children and Youth Version (ICF-CY) Model to a preschooler with DCD on evaluation, goal setting, and intervention. The case was a girl aged 5 years and 9 months who met the diagnostic criteria of DCD with normal cognitive development (cognitive developmental quotient 88 by the full-scale of Wechsler Intelligence Scale for Children) but impaired motor coordination (about 5 percentile as assessed by the Movement Assessment Battery for Children). We demonstrated the utilization of CMM in the processes of examination, evaluation, diagnosis, and prognosis prediction, and used the ICF-CY model for analyzing the child’s motor problems. Base on our analysis, the intervention goals for the child were to reduce falls and to improve self-care skills of the child, and to improve parenting skills. After two-month intervention, the goals were achieved. We suggest that physical therapists can utilize CMM and ICF-CY in assessment, goal setting, and intervention for children with DCD.
