背景:可靠的半定量分析方法有助於多巴胺轉運體影像的判讀,單光子斷層造影(SPECT/CT)系統可以 提供解剖學與功能性影像的準確對位。本硏究目的是藉由同一觀察者間與不同觀察者間的再現性來評 估在SPECT/CT系統中利用電腦斷層影像圏選R0I半定量分析紋狀體多巴胺轉運體攝取是否可行。
方法:共有四十三位臨床病人接受搭-99mTRODAT-腦部SPECT/CT檢查納入這項回顧性硏究。在電腦 斷層影像上手動圈選紋狀體、尾核、殼核以及枕葉皮質的R0I,然後將R0I轉移到SPECT影像上。以紋 狀體、尾核、殼核R0I的平均計數除以枕葉皮質R0I的平均計數分別計算攝取率(SUR、CUR、PUR)。
結果:使用電腦斷層影像圏選R0I方法,同一観察者間在SUR、CUR、PUR的差異平均分別爲3.04 土 2.82%、4.73 ± 4.71%、3.89 ± 3.24%,不同觀察者間在SUR、CUR、PUR的差異平均分別爲3.89 ± 3.18%、 5.48 ±4.36%、4.50 ±3.83%。Bland-Altman分析法顯示同一観察者間在SUR、CUR、PUR的誤差[一致性範 圍]分別爲0.02 [-0.09, 0.13]、0.03 [-0.15,0.21]、0.02 [-0.13,0.16],不同觀察者間在SUR、CUR、PUR的誤 差[一致性範圍]分別爲0.01 [-0.14,0.16] ' -0.03 [-0.23,0.17]、0.03 [-0.14,0.19]。
結論:在SPECT/CT系統中利用電腦斷層影像圈選R0I半定量分析紋狀體多巴胺轉運體攝取是一種具有 高度再現性的方法。
Background:A reliable method for semiquantifying striatal dopamine transporter (DAT) availability on DAT SPECT can reinforce the visual interpretation in clinical practice. A hybrid SPECT/CT system can provide accurate registration of anatomical and functional images. The aim of this study is to evaluate the feasibility of CT-based region of interest (ROI) for semiquantifying DAT availability in a hybrid SPECT/CT system by assessing its intra- and inter-observer reproducibility.
Methods:Forty-three clinical consecutive patients referred for Tc-99m TRODAT-1 brain SPECT/CT were enrolled in this retrospective study. ROIs of the caudate, putamen, and occipital cortex were manually delineated on the CT image and transferred to the SPECT image. The striatal, caudate, and putamen uptake ratios (SUR, CUR, PUR) were calculated as mean counts of striatal, caudate, and putamen ROIs divided by mean counts of occipital ROI, respectively.
Results:Using CT-based ROI delineation in Tc-99m TRODAT-1 brain SPECT/CT, intra-observer variabilities of SUR, CUR, and PUR were 3.04 士 2.82%,4.73 士 4.71%, and 3.89 士 3.24%,while inter-observer variabilities were 3.89 士 3.18%,5.48 士 4.36%, and 4.50 士 3.83%,respectively. Bland-Altman analysis showed that intra-observer biases [limits of agreement] were 0.02[0.09, 0.13], 0.03[0.15, 0.21], and 0.02[0.13, 0.16]; inter-observer biases were 0.01 [0.14, 0.16],0.03[0.23,0.17], and 0.03[0.14, 0.19], respectively.
Conclusion:CT-based ROI delineation for semiquantifying striatal DAT availability in a hybrid SPECT/CT system is a highly reproducible method.
