目的：以問卷探討求助自費心理諮商個案的問題類 型、動機及需求，作為後續推動心理衛生服務之參 考。
方法：以作者編製的問卷進行調查與資料收集，一共 收集35位當事人的填答問卷進行分析。
結果：女性82.9%，男性佔17.1%，年齡層以20-39歲 最多。求助心理諮商的問題類型以壓力與情緒控制 最多，佔37.1%，感情、婚姻問題佔25.7%，親子關 係問題佔20%。「直接、快速的接受臨床心理師服 務」、「費用合理、經濟上可以負擔」、「希望以 非藥物的方式進行諮商或治療」、「不紀錄病歷、 保護隱私」被85%以上受訪者認同與期待。
結論：以非藥物的方式進行心理介入、能直接、快速 的接受臨床心理師服務、過程重視隱私、費用合理 是看診民眾普遍且共同的需求。自費心理諮商門診 服務可以提供一般民眾另外一種求助管道，縮短民 眾的求助時間，避免醫療資源的浪費與社會成本增加。

Purposes：In this study a self-designed questionnaire was used to survey general hospital patients seeking psychological counseling not covered by health insurance. This included their major concerns about seeking help and their need for it.
Methods：A total of 35 completed questionnaires were collected and analyzed to gather data.
Results：Of the participants 82.9% were female and 17.1% were male; age ranged from 20 to 39. Among the major concerns, 37.1% were related to stress and emotion, 25.7% to marital problems, and 20% to parent-child relationship issues.
Conclusions
Over 85% of all participants supported receipt of immediate counseling services from psychologists, affordable counseling fees, psychological counseling with no drug treatment, and no medical record to insure confidentiality. Psychological counseling services not covered by health insurance could be available and effective resources for those needing mental health help.
