藉由問卷調查,了解放射線科全體人員對病人安全的態度,並探討安全文化與病人安全促進活動之間的聯動性。採用安全態度量表(SafetyAttitudesQuestionnaire, SAQ)於病人安全促進活動實施前後評量放射線科人員對病安態度的改變情形。SAQ 量表涵蓋五個主要構面,包括:「團隊合作」、「單位安全的風氣」、「對工作的滿意」、「對管理的感受」、及「工作狀況」。研究比較本科在不同構面的得分,並使用複回歸分析,探討影響構面分數的因子,最後縱貫性比較放射線科人員前後兩次測量構面分數的差異。前後測分別有84 及61 位受訪者,問卷回收率分別為86%及77.5%,SAQ問卷內部一致性Cronbach's alpha 分別為0.67~0.95 及0.66~0.92。擔任主管、年齡大於40 歲、工作年資大於10 年者都得到較高的分數。改善活動實施後,放射師於「團隊合作」、「單位安全的風氣」、「對工作的滿意」、「對管理的感受」和「工作狀況」等構面的得分皆有明顯的改善(73.3 vs. 83.8, 75.8 vs. 82.7, 70.1 vs. 79.4, 67.3 vs. 80.2, 71.4 vs.81.7, p < 0.05)。應用SAQ 問卷進行病人安全文化調查,可瞭解醫院內特定單位工作人員對安全文化的認知,也反映單位內安全文化可以透過改善活動的實施而提升。

英文摘要

The objectives of this study were to investigate the patient safety attitudes among staff of the radiology department using a structured questionnaire and to explore the impacts of quality and patient safety improvement activities on the patient safety culture. The Safety Attitudes Questionnaire (SAQ) was used to evaluate patient safety attitudes after implementation of improvement activities. The questionnaire consists of 5 dimensions of patient safety attitudes, including the teamwork climate, safety climate, job satisfaction, perception of management, and working conditions. Multivariate regression models were used to investigate the factors that may affect patient safety attitudes. In addition, we also compared scoring differences between the preand post-study periods. Eight-four and 61 objects participated in the pre- and post-study periods, and the respective response rates were 86% and 77.5%. Cronbach's alpha for the SAQ ranged 0.67~0.97 for the pre-study period and 0.66~0.92 for the post-study period. Staff with managerial jobs, aged more than 40 years-old, and having been employed for more than 10 years had higher scores in all dimensions. After the improvement activities were implemented, radiology technicians exhibited significant progress in all 5 dimensions of patient safety attitudes, including teamwork climate (73.3 vs. 83.8), safety climate (75.8 vs. 82.7), job satisfaction (70.1 vs. 79.4), perception of management (67.3 vs. 80.2), and working conditions (71.4 vs. 81.7; all p < 0.05). The SAQ survey data can reliably provide information of patient safety attitudes of a given unit within a healthcare organization as well as explore the effectiveness of quality and patient safety improvement activities on the patient safety culture within that unit.
