藥事服務是醫療服務重要的一環，藥事服務中之領藥服務，每個環節都環環相扣，疏失往往是一連串的失誤造成。因此如果能整合醫院資訊系統，讓病患在門診領取藥物後、透過自我的用藥檢查，或可提高病患多一份的用藥安全保障。某院因此開發條碼刷取式藥物辨識查詢系統，經由藥袋條碼刷取，就能在電腦螢幕上看到全部領用的藥品外觀及藥品使用資訊。該系統亦是國內應用條碼概念在藥品辨識的首次嘗試。但系統設立後，病人的使用情形卻不如預期，因此進行問卷調查。結果問卷的300 人中，僅87 人注意到醫院提供的條碼藥品辨識系統，亦僅19 人曾使用辨識系統。但經過藥師介紹後，有88.6% 的民眾認同刷條碼看藥品外觀及使用資訊的方式方便。81.7% 的受訪民眾表示以後會使用此系統，而民眾在領藥後審視藥物與閱讀藥袋之態度，與其使用意願有正相關。整體而言，民眾皆肯定系統的服務與其與藥事安全間之關係，對提供此項服務的滿意度亦高。

Medication errors can occur at any point in the medication delivery process, even with double checking by pharmacists. Most medication errors are not caused by carelessness, but rather by faulty processes or conditions that lead people to make mistakes or fail to prevent them. If the user (i.e., the patient) of the medication can do more than just take away the drugs they receive, it is possible to reduce such errors and reinforce patient safety.The studied hospital developed a bar-code medicine identification system. There is a barcode printed on the top of each drug bag, and the patient can use a bar-code machine to easily check the drug's picture. Hence patients can check if the medicine they received is the same as the picture shown on the screen.In a questionnaire interview of 300 patients, we found out that only 0.6% had used the machine. However, after a brief introduction to the system, 81.7% of those interviewed showed apositive attitude toward using the system in the future.
