In 1998, Longo described a method by which prolapsing hemorrhoids could be treated with acircular stapler, obviating the excision of either the anoderm or perianal skin. He postulated thatthe hemorrhoidal symptoms were primarily related to prolapse of the rectal mucosa and anoderm.The procedure for prolapsed hemorrhoids (PPH, a circular hemorrhoidopexy) resulted insimultaneous relocation and fixation of the hemorrhoids and anoderm. With the wideningacceptance of hemorrhoids as prolapsed anal cushions, a circular hemorrhoidopexy has become apopular procedure for the treatment of symptomatic hemorrhoids. Although laxity of Treitz'smuscles is a crucial underlying etiology of anal cushion prolapse, little emphasis has been placedupon them from the surgical perspective. With increasing acceptance of PPH, further attentionneeds to be paid to the surgical anatomy of the anorectal canal. This paper briefly reviews theanatomy and physiology of Treitz's muscles and reiterates their importance in the surgical setting.
