背 景:近年來乳癌躍升台灣女性癌症的首位,乳癌婦女在乳房被切除後接著化學治療時,除了會產生藥物副作用 帶來的身體不適外,還經常產生失落、無望等負面情緒,造成人際互動困難,生活滿意度及希望感降低的 情況。 目 的:探討靈性懷舊小組活動對於改善乳癌化療後婦女生活滿意度及希望狀態的成效。 方 法:採類實驗雙組前後測設計,以方便取樣招募乳房切除術後已接受化學治療的病人共39位(實驗組19位,對照 組20位)。兩組均在第一及第六週填寫基本資料、生活滿意度量表及赫氏希望指標量表,實驗組參加每週一 次,每次一小時為期六週的靈性懷舊活動。研究結果以廣義估計方程式針對各研究變項分析為期六週的活 動成效,選用主動迴歸工作相關矩陣控制時間的作用,並以強韌標準誤計算顯著性。 結 果:兩組在生活滿意度量表前後測總分改變情形,無明顯差異存在。但在赫氏希望指標前後測總分改變,有顯 著的差異存在。表示此項為期六週的的靈性懷舊小組活動對於乳癌化療後婦女的希望狀態足以產生良好的 成效。 結 論:靈性懷舊小組活動可作為乳癌化療後護理照護措施的參考。

英文摘要

Background: Breast cancer has risen to become the most common form of cancer in women in Taiwan. The side effects of medications used in radical mastectomy and chemotherapy cause physical discomfort and often cause negative emotions such as loss and hopeless, complicated interpersonal relationships, and reduced life satisfaction and hope. Purpose: This study explores the efficacy of spiritual reminiscence group activities on improving life satisfaction and hope in breast‐cancer and post‐chemotherapy women. Methods: A quasi‐experimental, two‐group pre‐and posttest design was used. Convenience sampling recruited a total of 39 post‐mastectomy patients who were currently receiving chemotherapy, with 19 assigned to the experimental group and 20 assigned to the control group. Both groups completed a life satisfaction scale and the Herth Hope Index Scale during the first and sixth week of the study. The experimental group participated in a one‐hour spiritual reminiscence small‐group activity on a weekly basis for six weeks. Generalized estimating equation analysis was used to analyze results data. Time of the correlation matrix effects was controlled, and robust standard errors were calculated for statistical significance. Results: No significant intergroup differences between pre‐ and post‐intervention life‐satisfaction‐scale scores were found. However, significant intergroup differences were found in scores on the Herth Hope Index Scale. The six‐week spiritual reminiscence small‐group activity in this study significantly increased hope in experimental group participants. Conclusions: Spiritual reminiscence small group activities may be used in nursing interventions to improve the spiritual wellbeing of breast cancer women after chemotherapy.
