背景:糖尿病病人接受胰島素治療對其血糖控制相當重要,而筆型胰島素注射器因為攜帶便利且適用於無法正確 辨識胰島素刻度之病人而普遍被運用。在有限的護理人力之下,有效益的多媒體影音光碟護理指導方式, 可以提升病人對筆型胰島素注射之能力與遵從度。 目的:探討「筆型胰島素注射多媒體影音光碟」的介入護理指導,是否比傳統護理指導在提升糖尿病病人對注射 技巧認知、縮短臨床護理人員護理指導時間、血糖控制與滿意度等方面有更佳的成效。 方法:採類實驗研究設計,將糖尿病病人分為(1)多媒體影音光碟配合回覆示範教學的實驗組(n = 40),與(2)傳統 書面護理指導單張配合口頭護理指導的控制組(n = 40),共收案80人。 結果:多媒體影音光碟護理指導與傳統護理指導對病人注射技巧認知及血糖的控制雖無顯著成效差異,但多媒體 影音光碟護理指導組的護理指導滿意度較高(t = -16.765, p = .000),且能顯著減少臨床護理人員護理指導的 時間(t = 3.60, p = .001)。 結論與建議:在護理人力不足與繁重工作負荷下,多媒體影音光碟護理指導能於有效時間內,達成相同的照護效果 又能提升學習者的滿意度。因此,本研究之成效,可以作為各醫療院所在護理人員進行糖尿病病人使用筆 型胰島素注射技巧之護理指導活動規劃與設計之參考。

英文摘要

Background: Insulin therapy is critical in the care of diabetes patients. The insulin pen has become popular due to its convenience and simplicity of implementation. Effective education methods are necessary to improve patient competence and compliance in using the insulin pen. Purpose: This study studies the comparative efficacy of using a multimedia CD‐ROM and using a traditional insulin pen injection on injection competency, teaching time, blood glucose control, and patient satisfaction. Methods: A quasi‐experimental design was conducted. Subjects were divided into the experimental group (multimedia CD‐ROM) and the control group (traditional education group), with 40 subjects in each group. Results: Our study showed no significant difference between the two groups in terms of injection competency, blood glucose control, and glycosylated hemoglobin (HbA1C). However, experimental group patients indicated higher satisfaction (t = ‐16.765, p = .000) than their control group peers. In addition, the multimedia CD‐ROM education significantly reduced the time spent on health education by nurses (t = 3.60, p = .001). Conclusions: In light of the current nursing shortage and the heavy workloads of nurses, using multimedia CDROMs for patient education is not only cost‐effective but also may improve patient satisfaction. Healthcare institutions may use the findings of this study to plan patient education related to insulin‐pen injections.
