背景:氰基丙烯酸酯是是屬於丙烯酸的樹脂產物,即市面上俗稱的瞬間膠,也可常見應用於醫療。近年來,氰基丙烯酸酯誘發氣喘的病例報告逐漸被發表,該類氣喘的發生大部分被認為是由於引發非IgE媒介致敏現象,導致遲發性喘鳴反應發生,但致病機轉仍未明確且診斷不易。本文將報告一因氰基丙烯酸導致職業性氣喘個案,並回顧職業性氣喘相關文獻,期望可提供臨床醫師早期診斷及處置的方法。材料和方法:本文為一位49歲於汽車工廠從事裝潢工作5年的男性,近年來開始有咳嗽與呼吸困難的症狀,症狀持續存在未改善而致醫院求治,進行身體檢查、實驗室檢查及檢測工作和休假時的呼氣峰流速(PEF)之肺功能。結論:職業性氣喘的診斷因其複雜性,常是醫學上一大挑戰,目前尚缺乏標準化診斷,患者離開誘發因子後,並不一定可以完全恢復正常,因此制定完整的診斷標準及檢查方式以協助早期診斷,不但有助於解決患者之臨床症狀,且可進一步保障患者健康,如此方能達到職業醫學上保護勞工健康的目標。

英文摘要

Background: Cyanoacrylate is an arylic resin, used in household use and medical. Recently, various cases of cyanocrylate-induced asthma have been reported. The sensitization mechanism is unknown, but the allergy is believed to be due to a non-IgE mediated phenomenon. A case of occupational asthma caused by cyanocrylate is reported in this article. We also review relevant literature on occupational asthma. Hopeful, it provides physicians ways to early diagnose and manage occupational asthma. Material and methods: We present the case of a 49-year-old male who developed occupational asthma (OA) due to cyanoacrylate while working in a car company for 5 years. For the previous years he had experienced periods of persistent cough and dyspnea. Physical examination, biochemical examination and peak expiratory flow (PEF) was measured during work and holiday to confirm the diagnosis of OA. Conclusions: Occupational asthma had no standardized diagnostic criteria make it difficult to diagnose early. Prevent further contact with allergen.
