背景與目的:有關輪椅乘坐不舒適性問題的探討仍有限,本研究欲了解各項人口學及臨床特性對身障者乘坐輪椅時,對整體不舒適性的關係。方法:受訪對象來自雲嘉地區身障團體與某區域教學醫院接受復健治療的身心障礙者,年齡介於20到85歲之間的身心障礙者,共67人,以TAWC(Tool for Assessing Wheelchair disComfort)做為評估不舒適的工具,分為「個案基本資料與輪椅使用現況」、「一般性不舒適度評估」、「不舒適強度等級」三部分。結果:研究發現輪椅乘坐不舒適性在教育程度、疾病類型、是否量身訂製上有顯著差異,且教育程度在高中以上的病患接受專業諮詢和量身訂製的比率,顯著高於國中以下者。男性病患接受量身訂製的比率顯著高於女性病患。多元迴歸分析結果顯示,教育程度在高中及以上者整體不舒適程度比國中及以下者高;脊髓損傷者的整體不舒適程度比腦中風患者高;曾接受過量身訂製者,其整體不舒適程度都比未接受者低。結論:本研究指出,應針對身障者狀況,提供輪椅使用與訂製之專業諮詢與衛教,以降低輪椅使用上的不舒適性及輪椅棄用的問題。

英文摘要

Background and Purpose: Information about the discomfort of using wheelchairs in persons with disability is very limited. The purpose of this study was to understand the association between the overall discomfort while using wheelchairs and the demographic and clinical characteristics of persons with disability. Methods: A total of 67 participants with disability were recruited from two disabled groups in Yunlin-Chiayi areas and from those who were receiving physical therapies in a regional teaching hospital. The age range of participants was between 20 to 85 years old. The modified TAWC (Tool for Assessing Wheelchair disComfort) was used as the tool for evaluation of discomfort. The TAWC included three sessions: demographics and wheelchair usage, general discomfort evaluation, and degree of discomfort intensity. Results: Our study found that there were three significant factors affecting the discomfort of using wheelchairs: educational level, diseases category, and custom made wheelchair service. Compared with those with junior high school education or and below, the disabled persons with senior high school education and above had a significantly higher percentage in using professional consultation and custom made wheelchair service. The male subjects had a significantly higher percentage than the female subjects using custom made wheelchair service. Multivariate regression model showed that the overall discomfort of subjects with spinal cord injury was higher than that of the stroke subjects. Overall discomfort of using wheelchairs was much lower in the subjects who used custom made wheelchair service than those subjects who did not use the service. Conclusion: Our study showed that providing professional consultation and education based on the condition of disabled persons was related to reduced overall discomfort of using wheelchairs and the wheelchair abandoning problem.
